The Company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel.
Per vessel's SMS, form FI 1.12-F03 shall be used for hot work. additionally, the area must be cleaned in and place, special attention should be given that inflammable materials behind/under the welding object must be removed.
Vessel crew utilized checklist FI-1.12-F03 prior to conducting hot work in cargo hold2, however they indicated that all combustible material was removed although there was still saw dust present that was used to clean up a transformer oil leak. The heat from the TORL A ignited the saw dust creating a fire onboard the vessel.
Recommend an internal audit be conducted within 90 days.

Cargo securing manual indicated containers in bay 39-41 were to be connected to the deck with twist-locks. PSCO observed containers not secured to the deck by twist-lock.

They shall also be permanently marked on the sides of the ships to the satisfaction of the Administration. The marks shall be plainly visible and, if necessary, special arrangements shall be made for this purpose.
Deck line was not a contrasting color from the hull and not plainly visible.

Vessel lost propulsion after the main engine went off line for a period of approximately 10-15 minutes while transiting up the Delaware River.
