After any survey of the ship under paragraph 1 of this regulation has been completed, no changes shall be made in the structure, equipment, systems, fittings, arrangements, or material covered by the survey without sanction of the administration. PSCO observed a valve installed on the sample line between the OWS and content meter. This valve was not on the line diagram provided by the crew.

Before the ship leaves the port and at all times during the voyage all lifesaving appliances shall be in working order and ready for immediate use. PSCO observed lifeboat release cover broken allowing water to enter the lifeboat.

Resolution: Upon initial investigation the release coverwas deteriorated and unableto maintain watertight integrity. The crew replaced the cover and demonstrated the watedight integrity. Liberian approved lifeboat technician approved the repair as fit for purose.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. While being directed by local Pilot onboard to perform astern movement (last command of evolution), M/E did not respond. Initial suspected cause, as indicated by C/E, is that there was not enough air in air reservoirs to complete command. 74SOLAS(14) I/11, 60AC

M/V Antaeus was reported to have been in collision with a barge while anchored at AMA anchorage. PSCO found a small inset approximately 1m x 1m in the starboard quarter near frame number 12. SOLAS 74 REG I/11
