PSCO observed #1 and #4 framo powerpack valve handle sensor being bypassed. Crew taped a bolt onto the handle which tricked the sensor to indicate that the valve was open when it was actually closed.

Resolution: Examination and necessary performance test carried out with attending surveyor and all found in good condition. In addition, as the reason for the above condition, general examination carried out and all found in order.
