In each crew berthing areas and washing areas information on the procedures and resources to report crimes, including sexual assault and sexual harassment, must be adequately posted. These procedures and resources must include at a minimum the following information: 1. Company policies regarding sexual assault, sexual harassment, and drug and alcohol usage. 2. Telephone numbers, websites and email addresses for reporting allegations of sexual assault and sexual harassment to the Coast Guard. 3. Company procedures to report violations of company policy and how to access resources. 4. Resources provided by outside organizations such as sexual assault hotlines and counseling. Provide a copy of informational bulletins addressing policies and procedures per safety management system satisfaction of attending Marine Inspector and post in crew staterooms and washing areas. 46 U.S.C. § 11101 07MAR2024: Received proposed SASH signage and extended deficeincy due date until 30APR24 to verify placement onboard once inport.

The owner of a vessel subject to inspection under section 3301 shall ensure such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to an established list of crew, by position, allowed to access and use the master key. This list must be maintained onboard the vessel and included within vessel’s safety management system. Additionally, a log book must be maintained with records of access and use of the mater key and shall include the following information at a minimum; date and time of access, room or location accessed, and name and rank of crewmember using the master key. This log book is required to be included in the safety management system. It may be in electronic format and shall be located in a centralized location that is readily accessible to law enforcement personnel. With respect to sexual harassment and sexual assault, procedures for and annual training requirements for all responsible persons and vessels to which this chapter applies on: Prevention; bystander intervention;reporting; response; and investigation. Incorporate the sexual assault and sexual harassment requirements into the Safety Management System to the satisfaction of the attending Marine Inspector. 46 USC 3203 46 USC 3205 07MAR2024: Received proposed SMS SASH response procedures extended def to 30APR24 to verify final incorporation into SMS.

The statutes relating to inspection of public vessels are primarily contained in 46 U.S.C. 2101(24) and 2109. a. 46 U.S.C. 2101(24) defines a public vessel as one that "is owned, or demise chartered, and operated by the United States Government or a government of a foreign country; and…is not engaged in commercial service." The SIKULIAQ is owned by the National Science Foundation (NSF), is operated by the University of Alaska, and is not engaged in commercial service. Government owned vessels as defined in 46 U.S.C. 2101(24) operated for oceanographic research are not required to be inspected and certificated by the Coast Guard. However, such vessels may be inspected and certificated upon request by the parent agency and upon an “interagency agreement to this effect.” Without an interagency agreement, the Coast Guard does not have authority to conduct inspections, or casualty investigations on the SIKULIAQ. ou are required to "establish" an interagency agreement between the National Science Foundation and the United States Coast Guard no later than February 4, 2023. Failure to establish an interagency inspection agreement with the Coast Guard prior to February 4, 2023 will result in the vessel being deactivated in the Coast Guard's database. In addition, Coast Guard Sectors Puget Sound and Columbia River will not conduct inspections on the SIKULIAQ without an interagency inspection agreement. The SIKULIAQ does not need a COI to operate as a Government owned Oceanographic Research Vessel. Failure to establish an inspection agreement will not limit vessel operations as a research vessel. It is our understanding that NSF owns several research vessels and is constructing new vessels that are operating, or will be operating in a similar manner. It would be beneficial to enter all NSF owned research vessels under the same interagency agreement. If you disagree with this decision you may ask for reconsideration, or appeal this decision in accordance with 46 CFR 1.03-20.

Make permanent repairs to #2 SSDG and/or replace in kind to the satisfaction of that attending USCG marine inspector and ABS.

Provide copy of approved plans and documentation of compliance inspections for installation of new compressed air system as required by 10-82 issued to vessel on 21OCT2019 for delegation of this oversight activity to be conducted by the American Bureau of Shipping. This shall be provided prior to placing new installation of compressed air system into service. 46 CFR 189.55-10 46 CFR 189.55-1

Provide pressure relief valve for new installation of service air pressure vessel not exceeding the MAWP (300 PSI) prior to placing into service. Current pressure relief is marked with set pressure of 320 psi. 46 CFR 54.15-10

Provide required markings for all general emergency alarm signal locations as required by 46 CFR 113.25-20 (c) “GENERAL ALARM - WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATIONS”. 46 CFR 195-05-1 46 CFR 113.25-20 (c)

Provide required markings for CO2 alarms in EDG and Incinerator room as required by 46 CFR 196.37-9. “WHEN ALARM SOUNDS VACATE AT ONCE. CARBON DIOXIDE OR CLEAN AGENT BEING RELEASED.” 46 CFR 196.37-9

Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in a serious marine incident within 2 hours. the vessel's alcohol test strips were expired. 46CFR4.06-3a

All general alarm bells shall be identified by red lettering at least 1/2 inch high: "GENERAL ALARM- WHEN BELL RINGS GO TO YOUR STATION". Vessel doesn't have any General Alarm signs. 46CFR196.37-7

Cylinders must always be secured and when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. Cylinders were found not properly stowed.

All ships of 1600 gt or more shall have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow and fill pipe, that has a capacity of one barrel. The containment around the hydraulic oil fill pipe was not attached to the deck, nor is it equal to one gallon. 33CFR155.320(a)2

Each fixture and lampholder must be fixed. The light located at he top of the ladder well, leading from the gear locker to the fore peak was not installed correctly. 46CFR111.75-20(d)2

remoted propulsion control station on bridge can take control from ECR without ECR pressing transfer button. Only ECR should have this capability. Verify ALL remote control stations do not have this function enabled and correct if needed.

Bowthruster charger/rectifier, primary power source of power loss not alarming on IMACS in ECR as indicated in the PSTP. Failure of normal source of power must alarm in a manned machinery space.

Generator prime movers must meet section 58.01-5 and 46CFR subpart 58.10. A small generator onboard experienced a thrown rod and incurred damage in the form of two holes in the crank case. Understanding that repairs can effectively be made in Seattle, WA, provide a comprehensive repair plan prior to departure from Dutch Harbor, AK.
