Cylinders must always be secured and, when not in use, they must be stowed in a rack in an up right position, with the valve protection cap in place.
PSCO observed 24 bottles (12 oxygen, 12 acetylene) stored on deck, tied to the handrail.

Resolution: 24 Oxygen and Acetylene bottles in question were removed from the vessel. Delivery receipt and Class report attesting to the same included in documents.
The steering gear compartments shall be: provided with suitable arrangements to ensure working access to steering gear machinery and controls. These arrangements shall include handrails and grating or other nonslip surfaces to ensure suitable working conditions in the event of hydraulic fluid leakage.
PSCO observed handrails in the steering compartment and lack of suitable nonslip surfaces.

The compant and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation...the code shall be treated as mandatory.
The company shuold identify equipment and technical systems the sudden operational failure of which may result in hazardous situations...that are not in continuous use.
Vessel was unable to provide proper documentation and procedures for safety pin arrangement in fixed CO2 system. Company shall conduct internal audit, focusing on fixed fire fighting systems maintenance.

Each transfer pipe system must not leak under static liquid pressure at least 1.5 times the MAWP. The test must be conducted annually. The vessels crew pressure tested the bunker line in the last year but used only air as the medicum. Conduct test in accordance with 33CFR156.170 prior to bunkering in U.S. waters.
