PSCOs discovered several corroded and wasted cable runs pipes and vents leading into cargo holds, therefore compromising the watertight integrity of the spaces. Identified a vent pipe was completed wasted to the extent it could be moved by hand, causing exposure of a 3-inch hole at the bottom of vent.

PSCOs observed multiple Lifebuoys had worn and unserviceable retro-reflective tape. Furthermore PSCOS observed lifebuoy grablines in unsatisfactory condition. Grab Lines appeared to be dry rotted and were able to be pulled apart by hand.

The vessel failed to fully establish maintenance to ensure the ship is maintained in conformity with the provisions of the relevant rules. This is made evident by deficiencies 02-05. Ships maintenance records state that the emergency generator and rescue boat steering were tested satisfactorily by the ships crew six days prior. Deficiencies 06-08 show that the vessel did not meet the requirement to provide for safe practices in ship operation and a safe working environment. An external audit is required within 90 days.

Vessel reported a large leak on deck in the A/C room in February 2018 and was corrected in March 2018. Since then, there is still a leak int he same space that has not been reported again and the vessel is using a wooden pallet to transit the space.

All electrical apparatus shall be so constructed and so installed as to not cause any injury when handled or touched in the normal manner. PSCO observed excessive water on the deck of the A/C room. There is a large pool of water under electrical boxes and near wiring. The vessel is using a wooden pallet for crew to stand on when working in the space. There is also a hole drilled in the bulkhead below the door in order to drain the space. The water is also causing excessive wastage to the CO2 cylinder base for the paint locker.
