The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended asn shall be so installed and protected as to reduce to a minimum any danger to persons onboard, due regard being paid to moving parts, hot surfaces and other hazards. PSCO conducted damage survey following propulsion irregularity and issue of COTP 0205-21 dated 06APR2021. C/E stated engine lost propulsion due to start air being fully exhausted following aggresive ahead and astern commands.

Resolution: Recieved, reviewed and accepted class report from LR stating loss of propulsion was due to inadquete air pressure in the start air recievers.Engineers recovered air reservoirs. Engine tested 12 times satisfatory in presence of port stae control officers.
