In the case of combination arrangement using an accomodation ladder with a trapdoor in the bottom platform (i.e. embarkation platform), the pilot ladder and manropes shall be rigged through the trapdoor extending above the platform to the height of the handrail. PSCO observed no manropes rigged through the trapdoor. 74 SOLAS (2020) V/23.3.3.2.1

01315 - Oil record book - Any failure of the oil filtering equipment shall be recorded in the Oil Record Book Part 1. PSCO observed that the memory for the Oil Content meter was missing for 04Mar19. C/E stated "battery was discharged and did not record". No entry was found in the ORB about in-operational oil filtering equipment. A corrective action plan is required in 30 days with concurrence from flag. Marpol Annex I/17.5
