Fire detection systems should be kept in good order so as to ensure their required performance if a fire occurs. "Universal drain box" in cargo hold smoke extraction system #8 for and aft / #4 mid have cracked housings that would render the system ineffective. Required monthly checks of a system were carried out in September without discovering.

All steering gear components and the rudder stock shall be of sound and reliable construction. Local control of the #2 stearing gear pump in the non follow up mode, is shuttering back and forth rendering stearing inoperable. All other steering gear pumps are in good working order, including the #3 which is the emergency pump.
