The Company should ensure that all personnel involved in the Company's safety management system have an adequate understanding of relevant rules, regulations, codes and guidelines. While conducting a test of the fixed gas detection system ships personnel utilized cargo as a sampling media. Procedure # DEG0014C ( monthly check of fixed gas system) states that span gas shall be used. (Def #4) Ship personnel stated that the master , C/O, 2/O, 3/O joined on the afternoon of the 27NOV2020. The off-signing members departed the morning of 29NOV2020. Company procedures for the relief of personnel outlined in TMM QA System manual 5.1.1(c) states that the crew shall be provided with proper familiarization of their duties in relation to the ISM Code. There is objective evidence discovered in an expanded ISM exam revealed the following non-conformity; the company failed to fully implement the requirements of the SMS as evident by def # 2,3,4. An internal audit is required with in 3 months. 74SOLAS(14) IX/3 ISM Code 6.4

Resolution: Audit result: Based on the audit the ISM system was found well implemented and in the samples verified, one observation was issued, and no non-conformities were identified.

The condition of the ship and its equipment should be maintained to conform with the provisions of the code to ensure that the ship will remain fir to proceed to sea without danger to the ship or persons onboard. PSCO observed 3 inch X 1/2 inch rupture in the deck water spray system. Hole was located in piping above cargo tank dome #1. PSCO observed rupture occur when deck water spray was activated during exam. 74solas(14) vii/13.1 IGC Code (1993) 1.5.3.1

Resolution: The damaged pipe section was renewed. Recorded video of spray system under pressure, the photo of the replaced pipe and the master's statement dated 2020-12-27 confirmed that the ruptured pipe was replaced and tested satisfactorily.

A water spray system for cooling, fire prevention, and crew protection should be installed to cover exposed cargo tank domes, exposed on deck storage vessels for flammable or toxic products, cargo liquid and vapor discharge and loading manifolds and the area of their control valves, boundaries of cargo compressor rooms. PSCO observed inadequate coverage for tank dome 1,2,3 Port and Starboard manifold, deck tank, and above motor compressor room for deck water spray. Tanks 1 and 2 Port and Starboard manifold had 50% coverage. 74SOLAS (14) VII/13.1 IGC Code (1993) 11.3.1

Gas detection equipment should be so designed that it may readily be tested. Testing and calibration should be carried out at regular intervals. Initial testing of the fixed gas detection system resulted in 106% LEL. Subsequent testing resulted in 56% and 60% LEL. The crew re-calibrated the system with 35% Span Gas , resulting in 33% LEL. 74SOLAS(14) VII/13.1 IGC Code 1993 13.6.6
