The various tasks should be defined and assigned to qualified personnel. Vessel is absent of any procedures that address when and how to access water-tight hatches in the hull. There are also no procedures to address how, when and parameters of sea state to embark a pilot. Additionally, procedures on donning and employing safety equipment prior to opening water-tight hatches are absent. Recommend an external audit of the vessel's SMS.
