To assist in casualty investigations, ships, when engage on international voyages, subject to the provision of regulation 1.4 shall be fitted with a VDR (voyage data recorder). PSCO observed error codes "018", "022", and "174 on voyage data recorder indicating that back-up HDD and DRU were disconnected and cannot be reconnected. Prior to departure , vessel must send technician report to the Coast Guard verifying that VDR is fully operational.
