The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this subchapter is provided, maintained, and replaced as indicated. Main fire pump observed with excessive leak at shaft seal. Tighten packing or replace as needed to the satisfaction of the attending marine inspector.

It shall be the duty of the chief engineer when assuming charge of the boilers and machinery of a vessel to examine them thoroughly. If any parts thereof are in bad condition, the fact shall immediately be reported to the master, owner or agent, and the Officer in Charge, Marine Inspection. Bow thruster engine cooling valve was found inoperable during CG inspection. Repair or replace in kind to the satisfaction of the attending marine inspector.

Each outlet or junction box must have a cover unless a fixture canopy, switch cover, receptacle cover, or other cover is used. Junction box for starboard (forward O1 deck) embarkation light is damaged, replace box.

CG was notified stating there were two bent blades on the propeller after running aground yesterday. Conduct repairs and test to the satisfaction of attending CG Marine Inspector prior to carrying passengers.

Doors, other than those which are normally locked, such as from staterooms, fan rooms, lockers, etc., shall be of the self-closing type capable of closing against a 31/2 degree list. Both FWD & AFT doors on the 01 deck for the passenger lounge will not close all the way shut. Make repairs and test to the satisfaction of attending Coast Guard Marine Inspector.

Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Vessel has not completed all required PSTP's annually. NCDOT will need to coordinate with local marine inspection unit to complete the remaining PSTP's.

Self closing FWD door on passenger deck (01) will not close properly. Must make repairs and test to the satisfaction of attending CG Marine Inspector.

All installations must be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. STBD MDE has a coolant leak. Make repairs and test to the satisfaction of attending CG Marine Inspector.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Fuel filter was deteriorated, rusty and bulging. Change fuel filter.

Each lifeboat, rigid liferaft, and rescue boat, unless otherwise stated in this paragraph, must carry the equipment listed in this paragraph and specified for it in table 1 to this section under the vessel's category of service. First aid kit had several expire or missing items. Replace all expired and missing items as required in 46 CFR 160.041

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Emergency lighting in store room was not working. Make repairs and test to the satisfaction of attending CG Marine Inspector.

Each space used by passengers or crew must be fitted with lighting that provides for a safe habitable and working environment under normal conditions. 6 deck lights and 5 overhead lights were found not working. Make repairs and test to the satisfaction of attending CG Marine Inspector.

The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in a satisfactory condition. Self closing fire door #9 & #12 not properly sealing.

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. Instructions for pull box outside the E/R were found missing.

In addition to the emergency power sources required under part 112 of this chapter, each self-propelled vessel and each mobile offshore drilling unit must have at least two electric generating sources. Captain stated vessel loss power from #1 SSDG and the EDG automatically energized. Crew shifted power from EDG to #2 SSDG. Crew discovered o-ring from oil pump had failed which caused the #1 SSDG to secure. Replace oil pump o-ring with in-kind material and provide video to marine inspector that the #1 SSDG is fully operational along with attestation from the captain that repairs are complete.

Master reported a soft "Bump n Go" while entering to Cedar Island, NC while in service at approximately 1336. Carry out and provide divers survey with photographic evidence of any findings to the satisfaction of the Coast Guard.

Provide common talking means of two-way voice communication and calling among the navigating bridge, emergency control stations, muster stations, embarkation stations, and other strategic positions listed in § 113.30–5. Prove proper operation on all talk back speakers on vehicle deck.

Each self-propelled vessel must have duplicate light sources for the side, masthead, and stern lights. Forward upper mast light housing and lens were found damaged rendering inoperable during inspection. Renew in kind and prove proper operation.

Vessels crew reported a "Bump n Go" grounding ivo #9 Big Foot Slough during entry into Ocracoke, NC while in service. Vessel is authorized transit back to Cedar Island. During a boat drill the crew discovered the vessel's general alarm was not audible on all the passenger decks. Trouble shoot and provide proper repairs shall be to the satisfaction of the Coast Guard. In the meantime, all crew shall be notified about the discrepancy. The loud hailer system shall be used as notification in the event of a general emergency. It shall be tested prior to each trip to ensure proper operation until the general alarm is repaired. The secondary means shall be radios by all crewmen as per your plan. The inoperative general alarm shall be logged along with the mitigation procedures listed.

In the event of an accident to machinery tending to render the furthur use of the item unsafe until repairs are made. MI received notification vessel ran aground. After U/W, Capt. discovered vibration on the STBD MDE @ approx. 1400-1500 RPM's. Vessel is granted one time waiver with zero passengers to dead head to Manns Harbor for repair. Provide proof to attending MI repairs have been conducted.

NAVIGATION LIGHTS MUST MEET UL 1104. VSL PORT LIGHT WAS OPERATIONAL BUT BROKEN. VSL MUST PROVIDE A NAVIGATION LIGHT WHICH MEETS THE UL 1104 STANDARD. 46 CFR 111.75-17

VHF RADIOS MUST BE CAPABLE OF TRANSMITTING THE VSL MMSI AND GPS LOCATION. VSL RADIO COULD NOT BE TESTED TO VERIFY ITS TRANSMITTAL OF MMSI AND GPS LOCATION. VSL MUST PROVIDE A VHF RADIO WHICH CAN VERIFY INFORMATION IS BEING PROPERLY TRANSMITTED. 47 CFR 1085

DOORS WHICH ARE NOT NORMALLY CLOSED MUST BE ABLE TO BE REMOTELY CLOSED FROM THE OPERATING STATION. DOOR HOLDBACK LOCATED ON MAIN DECK IN THE AFTER PORTION OF THE SUPERSTRUCTURE IS INOP. VSL MUST EITHER REMOVE THE MAGNETIC HOLDBACK OR PROVIDE ONE THAT WORKS. 46 CFR 72.05(a)(9)(ii)

CABLING MUST BE SECURED AND SUPPORTED IAW IEEE 18.3.5. CABLING FOR OVERHEAD LIGHT FORWARD OF DOGHOUSE IS NOT SECURED. VSL MUST PROVIDE APPROPRIATE SUPPORT FOR CABLE RUN.

Prove proper operation of the Fixed Firefighting System as per discussion with QA IAW 46CFR76.

Submit updated PSTPs as per results of test which indicated missing information IAW 46CFR62.

Fire pull station in pilot house has a key broken off in the reset mechanism. This station is unable to be reset. Repair IAW 46CFR76.27-10.

Ensure first aid kit meets requirements in 46CFR160.041.

Prove proper operation of #1 Port SSDG. Generator not able to take load at time of inspection.

Install lagging on STBD MDE.

Install deck plate in engine room.

Submit updated Fire Control Plan to reflect change of new fire extinguishers IAW 46CFR75.45-1.

Conduct permanent repair on #1 Port SSDG. The #1 Port SSDG currently is operating with a spliced wire. Repair IAW 46CFR70.25-1.

Port MDE M/G oil PSI not reading properly in pilot house or control room. Local gauge is currently in use and operating satisfactorily. Repair IAW 46CFR58.

Oil leak on Port SSDG. Conduct repairs prior to carrying passengers IAW 46CFR71.25-46.

Capt Reported the port MDE overheat alarm was set off and the temp was fluctuating approx 25* on the mechanical gauge. RPMs were reduced from 1550 to 1250 which corrected the overheating issue. Vessel was granted a onetime transit from Swan Quarter to Cedar Island for troubleshooting and repairs. Provide causal analysis as to the cause of the failure and corrective actions taken. Vessel shall not engage in commercial operations until cleared by local USCG Marine Inspector in accordance with 46CFR71.55.

At approximately 1042 EST, MSD Fort Macon received a report that the vessel's STBD MDE shut down while carrying passengers from Ocracoke to Swans Quarter. 51 passengers, 07 crew and 27 cars on board. Trouble shoot/Repair and provide MI with tech report and CG-2692 IAW 46CFR58.10-10.

In addition to the emergency power sources required under part 112 of this chapter, each self propelled vessel must have two electric generating sources. NR1 generator is non operational due to a leak on the fuel pump. Vessel is permitted to a one time transit from Ocracoke to Cedar Island. Permanent repairs shall be conducted in Cedar Island to the satisfaction of the U.S. Coast Guard prior to departing Cedar Island.

The master reported a sustained reduction of speed on the #1 ships service diesel generator during service. Trouble shoot to determine cause of casualty and provide caterpillar technicians report with findings to the satisfaction of the Coast Guard.

Firefighting equipment shall be ready for use at all times and be maintained or replaced as indicated. The hoses for #15, #16 and #23 fire extinguishers were found wasted. Renew in kind.

The installation of lifesaving appliances must be in accordance with the requirements of subchapter W. Passenger deck portside life ring buoy water light lanyard is wasted.

Means shall be provided to close all vents. Vent closure for elevator shaft was not properly closing.

All watertight doors in subdivision bulkheads shall be kept closed during navigation. #5 watertight door was found opened with holdback.

Each launching appliances must be arranged so that persons onboard the vessel can safely and rapidly board the the survival craft. During drills MI observed two crewmembers received several cuts while retrieving the rescue boat. Recommendation to provide personal protection equipment.

The survey shall be such as to ensure that necessary repairs have been effectively made. Steering feedback unit arm is loose and missing hardware.

It shall be the duty of the master to see that the emergency lighting on a vessel is in proper operation condition. Light is dim on battle lanterns. Renew batteries as needed.

Vessels over 500 gross tons but not more than 1500 gross tons must be equipped with two independently driven fire pumps. The vessel reported a leak in the main fire pump's pump housing. Make permanent repairs and prove proper operation to the satisfaction of the USCG.

Vessel damaged port propellar during grounding, replace prop iaw 46 CFR 72.01-15.

Passenger deck stern port side deck light not properly sealed.

Passenger deck aft port side life jacket locker missing markings.

Main deck engine room vents missing three bolts.

Prove proper operations of general alarm. GA sounds in manual mode but not when cascading from fire system. 705C: 7 days

Bilge manifold suction valve for void (shaft alley) sucking air due to loose bolts not even hand tight. Could not get suction during operational testing.

Steering gear room bilge suction vlv seized/will not open or close at time of inspection.

Rescue boat cradle aft pin safety clip broken in half. Install new safety clip.

Remove all welding cable from electrical systems. Replace with authorized ship cable. MI found welding cable on batteries.

Remove wing nuts from batteries. Install proper hardware as discussed.

EDG oil pressure switch not properly mounted. Loose not bolted.

Fire door #9 handle not properly secured and loose.

Steering room gear room bilge alarm wire not properly secured and laying in the bilge.

Pilothouse door to passage way is sticking. Hard to open/close.

Port side rudder nut locking strap found not welded by MI during def check. Vsl authorized a one time transit to Ocracoke for permanent repairs.

MI found WTD to shaft alley from engine room with unapproved hold back, holding WTD in the open position.

The starboard rudder clamp to pintle was found fractured and damaged.

Master reported the crew discovered a small leak in the marine sanitation device, reported in vicinity of valve. The vessel was authorized a one time transit to deadhead back to Cedar Island, NC. Determine the source and complete proper repairs in-kind to the satisfaction of Coast Guard, prior to carriage of passengers.

Ensure rescue boat is rigged for immediate use. Remove line from remote pull. Total MOB time was 15min.

Center mast not under command light is not working. Replace damaged wiring and ensure proper working.

Conduct training with crew for MOB pointer position. Ensured pointer kept eye on MOB.

Prove proper operation on fwd void hatch.

Properly secure #25 CO2 bottle in tank room.

Renew bilge suction gauge.

Steering system is showing a five degree difference to port on #1 and #2 systems. Determine cause and make proper repairs.

The fire detection panel showed trouble on aft loop. Trouble shoot and determine proper repairs.

The self closing fire safety doors #9 and #10 were not closing properly and had loose hardware.

The pilot house window wipers were inoperative. Fair winds operation only.

The pilot house fire pump gauge was inoperative when tested.

Battle lanterns were not working in the port and stbd store room spaces.

The forward void is missing egress signage.

Bow thruster jacket water was leaking from sight glass.

Bow thruster room e-lights were not operable.

CAPT reported a loss of maneuverability to the Coast Guard following a failure of the bow thruster while docking in Okracoke, NC. Vessel is permitted to transit without passengers to Cedar Island to conduct further diagnostics and troubleshooting. All repairs are to e made to the satisfaction of attending marine inspector prior to carrying passengers.

The port and stbd MDE's were alarming on low oil pressure, during sea trials. Determine cause and make corrections per Mfg. spec's.

The #3 fire station vari-nozzle was damaged.

Three of the vessels life rafts were missing gromments properly sealing the painter lines in place.

Donning instructions for life jackets were missing in the aft stowage box.

Fire extinguisher #23 was loose and bottle was damaged. Replace or provide servicing.

Three Co2 stations were noted as loose and missing hardware at 25, 20, and 16.

The forward and aft cargo nets are damaged. Replace inkind.

The vessels chief engineer license did not meet the conditions of the manning per the COI. License presented - stated in limitations motor propelled for less than 500 GT, vessel is greater than 500. Provide prove of proper manning within 90 days, per CPREV's direction.

#1 SSDG lube oil switch was chaffed. repair or replace in kind.

M/I identified hold backs on engineering doors.

The bow thruster was alarming on low HPU intermitly. Determine cause. M/I tested full function of operation was noted this date.

Provide third party reports for EDG and engine room iaw PSTP's.

Means shall be provided to close off all vents and ventilators. Vent closures on the 03 and 02 deck need to have renewal and be chalk tested. Total of 05. 46 CFR 72.15-15

Lifesaving appliances shall be maintained IAW subchapter W. Davit falls are past due for renewal. Renew falls prior to carriage of passengers. 46 CFR 71.25-15

ENGINE ROOM TO SHAFT ALLEY WTD OBSERVED WITH A HOLD BACK INSTALLED.

Crew reported flooding on bow thruster on the aft flange on the bow thruster.

Replace #25 portable fire extinguisher mount on main deck.

The galley smoke detector was found damaged this date.

The fwd and aft bulkhead was missing 1' x 1' structural fire protection.

Adjust water tight service hatch to make watertight.

Replace missing items in first aids kits onboard.

The vessel bow thruster room escape hatch was leaking.

The vessel emergency anchoring system (wire rope) was wasted and was not being maintained.

The steering system in rudder room had two small hydraulic leaks.

Pilot house Fire ext. #24 found loose in bracket. Install proper fire ext with approved bracket. It appears the fire ext was changed out at somepoint.

Void below MOP Compartment midship remove water and check piping, Hatch gsk for leaks.

Bow Thruster WTD and WTH in overhead of aft compartment, dog's were not adjusted properly.

Vessels vehicle deck fixed fire system was inoperative when tested by crew, during this inspection.

Vessels has incorrect strainers on the fixed bilge water system. Modification or renewal to adaquate intake size is required.

Emergency generator fuel bowel has plastic bleed drain. Renew or replace with metal drain plug is required.

The #1 SSDG lube oil supply (hard) pipe fractured during transit.

Vessels #2 SSDG hardline that carrys oil to injectors had minor leak. Same line was cause of M/C on #1 SSDG.

Water tight door to Main Engineering was not watertight. Adjusted and corrected on the spot.

two leaking elbow joints were noted in the sprinkler system. One in E/R and one overhead on main deck IWO elevator enterance

Conduct hydrostatic test of entire sprinkler system to 1.5 x the MAWP. Multiple pinhole leaks have been discovered & repaired.

Rudder Angle Indicator at the helm had a 3 degree variance compared to actual rudder position.
