Whenever there is a hazardous condition either on board a vessel or caused by a vessel or its operation, the owner, agent, master, operator, or person in charge must immediately notify the nearest Coast Guard Sector Office, and in addition submit any report required by 46 CFR 4.05-10. The vessel arrived into Jamestown Anchorage loaded with diesel in all cargo tanks with an inoperable inert gas generation plant and cargo tanks that were above acceptable oxygen content; a condition that adversely affected the safety of the vessel, crew, and port. The two conditions were not made aware to the U.S. Coast Guard until the start of the Port State Control Examination at Jamestown Anchorage on 21 FEB 2022.
