A fire-protected lifeboat shall be capable of protecting persons from a continuous fire that envelops the lifeboat. Lifeboat water spray piping has broken loose from one of its stern pipe hangers, contacting the davit when the boat is retrieved. 74 SOLAS (20) III/31.1.7 Action: 50ac

Where the atmosphere in double hull spaces cannot be reliably measured using flexible gas sampling hoses, such spaces shall be fitted with permanent gas sampling lines. The configuration of gas sampling lines shall be adapted to the design of such spaces. While on board the vessel for subsequent examination on 22 FEB 2022, PSCO identified the fixed gas detection system serving double hull spaces and double bottom spaces to be in "fault" condition. 74 SOLAS (04) II-2/4.5.7.2.2

Whenever there is a hazardous condition either on board a vessel or caused by a vessel or its operation, the owner, agent, master, operator, or person in charge must immediately notify the nearest Coast Guard Sector Office, and in addition submit any report required by 46 CFR 4.05-10. The vessel arrived into Jamestown Anchorage loaded with diesel in all cargo tanks with an inoperable inert gas generation plant and cargo tanks that were above acceptable oxygen content; a condition that adversely affected the safety of the vessel, crew, and port. The two conditions were not made aware to the U.S. Coast Guard until the start of the Port State Control Examination at Jamestown Anchorage on 21 FEB 2022.

For tankers of 20,000 deadweight tons and upwards the protection of the cargo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the Fire Safety System Code. Vessel departed Rotterdam and arrived in the U.S. with an inoperable Inert Gas Generator. Cargo Tanks 1 P/S, 2 P/S, 4 P/S, 5 P/S were sampled and found above 15% oxygen in each tank. 74 SOLAS (04) II-2/4.5.5.1.1

Considering the deficiencies identified on the PSC Form B dated 20FEB2022 as objective evidence, that company has not established and carried out procedures as to ensure the ship's material condition is in conformance with the provisions of the relevant rules and requirements. All steering gear components and the rudder stock shall be of sound and reliable construction to the satisfaction of the administration. When steering from the bridge, the rudder is hunting by 5 degrees. 74 SOLAS (04) II-1/29.2.1

Other oil level gauges may be used in place of sounding pipes subject to the following condition. PSCO observed the sight glass lower gag broken and stuck in the open position; creating a potential fuel release into the space from the ME/DG expansion tank. 74 SOLAS (04) II-2/4.2.2.3.5.2

Fuel oil transfer pump discharge line (on gasket) found leaking with water bottle placed below to catch drips. The hydraulic tank filling line was found leaking with a water bottle catch basin to collect leaks. 74 SOLAS (04) II-2/4.1.1

A secondary means of allowing full flow relief of vapor, air, or inert gas mixtures shall be provided. Alternatively, pressure sensors may be fitted in each tank. Pressure sensor for #2S cargo tank found malfunctioning (-299 mBar). 74 SOLAS (04) II-2/11.6.3.2

Provisions shall be made to guard against liquid rising in the venting system. This shall be accomplished by high-level alarms or overflow system. High level indicator alarm for #1S cargo tank was inoperable. Additionally, #6P is inoperable. 74 SOLAS (04) II-2/11.6.3.1

Means shall be provided for measuring the cargo temperatures. Temperature sensor in #4P found malfunctioning. 74 SOLAS (20) VII/10.1 and IBC 7.1.5

Each transfer pipe system must not leak under static pressure. Bunker lines and cargo lines were tested only to MAWP. 33 CFR 156.170(c)(4).

The machinery shall be of a design and construction adequate for the service for which it is intended. Found a "C" clamp holding down the cover on the duplex fuel oil filter on the #2 generator. 74 SOLAS (04) II-1/26.1

The garbage record book. Each discharge shall be recorded in the garbage record book. PSC was provided a receipt from 11 May 2021 and no entry was made in the garbage record book for category A and C waste. On 8 October 2021, a discharge of category A, B, and C garbage was made in port and recorded, no receipt could be provided. MARPOL V/10.3.1

The arrangement for the storage, distribution, and utilization of other flammable oils shall be such to ensure the safety of the ship and persons on board. PSCO observed hydraulic storage tank sight glass gag held in place open with a rag; creating a risk of release of hydraulic fluid into the space. 74 SOLAS (04) II-2/2.4
