Cargo spaces shall be fitted with sample extraction smoke detections system complying with requirements of FSS code which states any required system shall be capable of continuous operation at all times. Vessel has valid Document of Compliance special requirements for ships carrying dangerous goods which indicates a sample extraction system is required on board. Port State Control Officer noted systems hoses are disconnected from control panel, rendering system non operational. Standard practice onboard consists of 1) Removing the F1 transformer fuse IAW operating instructions manual. 2) Disconnecting hoses when carrying non-dangerous goods as cargo. The manual makes no mention of removing hoses & vessel does not have procedures documenting the practice in the vessl's Safety Management System.

Fire control plans shall be kept up to date. Vessels DOC-Special Requirements for ships carrying dangerous goods requires a total capacity of 12kg of dry powder portable fire extinguishers per cargo hold in addition to any portable fire extinguishers required elsewhere by chapter II-2. The vessel has 06 cargo holds & locations for only 11 6kg portable extinguishers, instead of 12, to equal the required amount per hold. In addition, thier locations on deck are marked to indicate the portable extinguishers required by the DOC are carried as cargo. However, fire control plan indicates the extinguishers are located on deck with no provision for storage in relation to cargo carried on board.
