Every company should develop, implement, and maintain a safety management system which includes the following functional requirements: instructions and procedures to ensure safe operation of ship. Vessel failed to implement it's SMS w/respect to cyber risk management. Vessel displayed password to network connected printer and satellite link application, USB blockers not utilized, ship's crew connected personal phones to network, vessel used generic logon w/no passwords, server room door unlocked and unattended, ECDIS USB drive unencrypted and left in unlocked drawer, server OS no longer supported by Microsoft. Require internal audit to ensure SMS cyber training and hygiene implemented.

Materials readily rendered ineffective by heat shall not be used for fire mains and hydrants. Vessel had a branch line off the Fire Main in the engine room which had a small hole in it. The vessel had made a temporary repair to it with epoxy. Vessel should have permanent repair completed to the fire main to the satisfaction of the Administration or Class Society.
