Where the specific cargo is liable to damage the materials normally used in electrical apparatus, due consideration shall be given to the particular characteristics of the materials chosen for conductors, installation, metal parts, etc. As far as necessary, these components shall be protected to prevent contact with gases or vapors liable to be encountered. Four explosion proof lights were found on the cargo deck with loose wiring. Three of the lights had loose packing glands on the light fixtures. One light had a packing gland that was disconnected. Instead, the light and cabling was held together by blue electrical tape. Lights and cabling were not installed properly therefore potentially allowing for contact with gases or vapors. 74 SOLAS (14) VII/10.1 IBC Code 10.1.2.2 Ref IEC 60092-502: 1999

The company and the ship shall comply with the requirement of the international safety management code. And meeting the requirement, the company should ensure that any non-conformity is reported, with its possible cause, if known (10.2.2) and appropriate corrective action is taken (10.2.3) In accordance with the vessel safety management system QHSE 4.2.1 plan maintenance system, the primary objective of the PMS is to improve effectiveness of maintenance. In accordance with PMS job check explosion proof lighting equipment for component 850.95.08 (Explosion proof light), Check for proper sealing of cable glands. As delineated in deficiency one, objective evidence attest to the vessel’s crew failing to properly conduct maintenance and inspections of electrical equipment. An internal safety audit and corrective action within three months is reccommended. 74 SOLAS (14) IX/3.1 ISM Code 10.2

06199 - Each vessel vapor connection flange face must have a permeant stud projecting outward 1" long. It must be located at the top of the flange face, midway between bolt holes, and in line with the bolt hole pattern. Stud was not permanently affixed and was not located at the top of the flange face. 46 CFR 39.2001 (j).

Resolution: Ship provided photographs attesting to the repair of the port side aft vapor control line as well as photos of the remaining 3 vapor control lines to ensure proper placement and permanently affixed studs.

Cargo was identified in the #6 port ballast tank originating from #6 port cargo tank. Ballast tank was entered by ship's crew where 8-10 liters of cargo (gasoline) was found eminating from a possible crack on longitudinal bulkhead of #6 port cargo tank 4M forward of slop tank.
