Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station, conspicuously marked, and located at the midpoint of such vessel, or 100 feet from the engine, whichever is the more practical. The remote quick acting manual shutdown shall be installed on the engine so as to provide a quick and effective means of stopping the engine (such as by cutting off the intake air). MI did not witness satisfactory shutdown of the port internal combustion engine driven cargo pump. 46 CFR 32.50-35 (a)

Each hose also must be marked with the following, except that the information required by paragraphs (f)(2) and (3) of this section need not be marked on the hose if it is recorded in the hose records of the vessel or facility, and the hose is marked to identify it with that information:
(1) Maximum allowable working pressure;
(2) Date of manufacture; and
(3) Date of the latest test required by 33 CFR 156.170.
Annual hose pressure test certificate did not have serial numbers listed to identify which hose was tested. MI was unable to determine which hose was tested. A single 4" hose on deck was found with a test date of OCT 2020.

All Pressure vessels other than unfired steam boilers shall be protected by pressure-relieving devices that will prevent the pressure from rising more than 10 percent above the maximum allowable working pressure.
Vessel did not have correct size pressure relief valve installed on the pressure vessel.
