Every ship of 100 gross tonnage and above... shall carry a garbage management plan which the crew shall follow. PSCOs observed garbage being stored not in accordance with their garbage management plan. MARPOL Annex V / Reg 10.2

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs...fire detection and fire alarm systems. PSCOs observed that the smoke detectors in the lower engine room had been de-energized due to giving false alarms. 74 SOLAS (04) II-2/14.2.1.1

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. Crack found in fuel tank #4. SOLAS(04) II-1/26.1

Suitable Measure are taken for safeguarding independent emergency operations under all circumstances. Emergency generator may be used for short periods to supply non-emergency circuits. vessel routinely runs emergency generator to power refrigerated cargo.

The Life buoys complying with the requirements of paragraph 2.1.1 of the code so distributed as to be readily available on both side of the ship. the vessel had 02 lifebuoys missing from the assigned spots.

THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS TO ENSURE THAT THE SHIP IN ALL RESPECTS WILL REMAIN FIT TO PROCEED TO SEA WITHOUT DANGER TO THE SHIP OR PERSONS ON BOARD. VESSEL'S MAIN DIESEL ENGINE EXPERIENCED A LOSS OF PROPULSION.

To discharge ballast water into waters of the United States, the master, owner, operator, agent, or person in charge of a vessel subject to 33CFR151.2025 of this subpart must either ensure that the ballast water meets the ballast water discharge standard as defined in 33CFR151.2030(a), use an AMS as described in 33CFR151.2025(a)(3) or ballast exclusively with water from a U.S. public water system, as described in 33CFR151.2025(a)(2), according to the schedule in paragraph (b) of this section. Vessel implementation schedule to install a BWMS/AMS was the first scheduled dry docking after January 1, 2014. Vessel conducted a scheduled flag state and classification society dry dock for credit/survey on June 20, 2016 and no BWMS/AMS was installed. Vessel is prohibited from discharging ballast water in the waters of the United States. This deficiency is to remain in effect until one of the compliance methods is met in 33CFR151.2035.

0740- In Cargo Ships there shall be an alternate means consisting of an emergeny fire pump complying with the provisions of the fire safety systems code. Emergency fire pump in not able to maintain pressure in accordance with the provisions set forth in the fire safety systems code. 74 SOLAS (04) II-2/ 10.2.2.3.1.2

1599 - All ships of 300 gross tonnage and upwards irrespective of size shall be fitted with a speed and distance measuring device, or other means, to indicate speed and distance through the water. 74 SOLAS (2014) Ch. V/19.2.3.4
