System for continuous monitoring of the concentration of hydrocarbon gases shall be fitted on all tankers constructed before 1 July 2002. During testing of fixed system, hydrocarbon/H2S sensors failed & no readings. Servicing by authorized technician required to restore system function w/in 30 days. Manual readings of impacted spaces required during daily rounds/watches. The company and the ship shall comply with the requirements of the Int'l Safety Management Code. Company should identify technical systems which the operational failure of may create a hazardous situation. Reliability of systems should be ensured through regular testing to promote system reliability. Limited scope internal SMS root cause analysis w/ management recommendations (corrective action report) for testing checklist & guidance to assist w/ routine system testing for fixed hydrocarbon detection. Due w/in 30 days.

Maintenance, testing and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. Noted damage to bow of FRB w/ crack & water ingress into fiberglass. OEM servicing & authorized repair kit required w/in 30 days. 19JUN2024: CG-835V extended 30 days. OEM advised vessel of repair options and materials necessary. ABS Honolulu verified and interfaced w/ vessel management to discuss.

Officers and ratings assigned specific duties and responsibilities related to cargo or cargo equipment on oil or chemical tankers shall hold a certificate in basic training for oil and chemical tanker cargo operations. 3AE - no STCW basic oil and chemical tanker cargo ops training. Limited scope internal SMS root cause analysis due in 30 days. The company should ensure that each ship is manned with qualified, certificated and medically-fit seafarers in accordance with national and international requirements. 19JUN2024: Extended 30 days due to on-going USCG HQ determination (MMC Policy office)

The loadline mark shall be painted white or yellow on a dark hull and plainly visible. Loadline - starboard side - improper marking and paint. Correction due in 30 days.

Condition of ship and its structure shall be maintained. Following areas were identified during deck exam for corrosion and correction at next drydock (2025): 1. Piping penetration for cabling ivo FRB 2. Steam exhaust piping ivo port piping manifold/containment by exterior house - main deck.

Party shall ensure that ships entitled to fly its flag or operating under its authority are surveyed and certified iaw annex IV for compliance with IAFSC. “Anti-fouling system” means a coating, paint, surface treatment, surface, or device that is used on a ship to control or prevent attachment of unwanted organisms. Flag state observation of marine growth layers along exterior of hull ivo waterline indicating concerns w/ effectiveness of anti-fouling coat. Item to be documented via this CG-835V and correction at next drydock (2025).

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. MI observed that the limit switch on the rescue boat davit was not installed properly such that it engaged every time the boat was lowered and could not be raised. Ship's crew was able to adjust the limit switch and MI observed proper operation. 74 SOLAS (2020 cons.) III/20.2

The requirements of § 58.10–5 (a), (c), and (d) shall apply to diesel engine installations...All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Vessel has fuel oil leaking out of the lube oil cap on emergency generator. Vessel experienced a loss of propulsion momentarily when the vessel did not respond to the stern bell. Must demonstrate corrective action to the satisfaction of class and the Coast Guard

Cargo tanks shall be fitted with a visual and audible high-level alarm; level alarms shall be capable of being tested prior to loading. The high level alarm on the #4 port side tank is inoperable; demonstrate proper operation prior to loading cargo in the #4 port side tank.

Waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottoms. Paper bags were being used as waste receptacles throughout the ship.

Incinerator refractor cracked. Requires repair.

Bilge and Sludge systems have pneumatic diaphram pump installed. Vessel diagrams do not reflect this configuration. Vessel must modify drawing to reflect the diaphram pump and attest approval from the U.S. Coast Guard. Drawings to be submitted to U.S. Coast Guard.

Vessel reported a reduction of propulsion due to failure in the #1 cylinder not allowing vessel to exceed half ahead. The design and arrangement of main propulsion machinery must be equivalent to the standards established by the ABS Rules. Vessel shall provide tech report identifying cause of failure and satisfactory means of making repairs to certificate issuing authority and USCG.
