33 CFR 155.1060(b): Annually, at least one of the exercises listed in § 155.1060(a) (2) and (4) must be unannounced. An unannounced exercise is one in which the personnel participating in the exercise have not been advised in advance of the exact date, time, and scenario of the exercise. Vsl unable to produce documentation of unannounced response plan exercise.

33 CFR 155.1060(a)(4): Oil spill removal organization equipment deployment exercises, which must be conducted annually. Vsl unable to produce documentation of equipment deployment exercises.

33 CFR 155.1060(a)(1): Qualified individual notification exercises, which must be conducted quarterly. Vsl unable to produce documentation of quarterly QI notifications.

33 CFR 155.1060(a)(2):Emergency procedures exercises, which must be conducted quarterly. Vsl unable to produce documentation of quarterly emergency procedure exercises.

33 CFR 155.1055(b): A vessel owner or operator shall ensure the maintenance of records sufficient to document this training and make them available for inspection upon request by the Coast Guard. Records must be maintained for 3 years following completion of training. 3 years of response plan training records not available to USCG upon request.

33 CFR 104.230(b)(1): The VSO must ensure that at least one security drill is conducted at least every 3 months. Vsl unable to produce documentation of required security drills from the past 2 years.

46 CFR 58.10-5(a) : All installations must be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Exhaust manifold on cargo pump engine was heavily corroded.

33 CFR 156.170(c)(2): Each transfer system relief valve must open at or below the pressure at which it is set to open. Cargo pump relief valve did not have record of annual testing.

33 CFR 156.170(c)(3): Each pressure gauge must show pressure within 10 percent of the actual pressure. Transfer system pressure gauge did not have record of annual testing.

33 CFR 155.400(e): Each hose must be marked with one of the following: The name of each product for which the hose may be used; or “OIL SERVICE”. Cargo transfer hoses were not marked with service type.

33 CFR 155.400(f): Each hose also must be marked with the following, except that the information required by paragraphs (f)(2) and (3) of this section need not be marked on the hose if it is recorded in the hose records of the vessel or facility, and the hose is marked to identify it with that information: (1) Maximum allowable working pressure; (2) Date of manufacture; and (3) Date of the latest test required by 33 CFR 156.170. Transfer hoses were not marked IAW regulations.

33 CFR 104.120(b): Each owner or operator of an unmanned vessel subject to this part must maintain the documentation described in paragraphs (a)(1), (2), or (3) of this section. The letter required by each of those paragraphs must be carried on board the vessel. No VSP approval letter was on board vessel.

Each transfer pipe system must not leak under static liquid pressure at least 1 1 2 times the maximum allowable working pressure. Cargo piping system did not have record of annual testing at 1.5 x MAWP. 33 CFR 156.170(4) Prior to cargo operations.

Not burst, bulge, leak, or abnormally distort under static liquid pressure at least 11 2 times the maximum allowable working pressure. Transfer hoses did not have record of annual testing at 1.5 x MAWP. 33 CFR 156.170(1)(iii) Prior to cargo operations.

Vent outlets from all tanks which may emit flammable or combustible vapors, such as bilge slop tanks and contaminated drain tanks, must be fitted with a single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two screens of at least 20 by 20 mesh spaced not less than one-half inch (13mm) nor more than 1 1/2 inches (38mm) apart. The clear area through the mesh must not be less than the internal unobstructed area of the required pipe. 5 out of the 6 cargo tanks goose neck vents had rips/holes in the mesh of the flame screen. Vessel is require to replace the flame screen mesh to the satisfaction of the attending MI. 46 CFR 56.50-85(a)(8) 16C

Valves for drawing fuel or draining water from fuel are not permitted in fuel oil systems except that a single valve may be permitted in the case of diesel driven machinery if suitably located within the machinery space away from any potential source of ignition. Such a valve shall be fitted with a cap or a plug to prevent leakage. Vessels fuel oil filter on the prime mover had a plastic collection bowl and plastic valve. Vessel is required to replace or fit the prime mover with the appropriate items for the fuel filter to the satisfaction of the attending MI. 46 CFR 56.50-60(f) 16C

Each cable installation must meet— (1) Sections 6, of IEEE 45.8–2016 (incorporated by reference; see § 110.10–1 of this subchapter); or (2) Cables manufactured to IEC 60092–353:2016 must be installed in accordance with IEC 60092–352:2005 (both incorporated by reference; see § 110.10–1 of this subchapter), including clause 8. Vessels battery cables on the prime mover were cracking/brittle wiring insulation. Vessel is required to replace the defective battery cables with approved battery cables to the satisfaction of the attending MI. 46 CFR 111.60-5(a) 16C

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date, except as specified in paragraph (b) of this section. Vessel can provide no evidence that it underwent its first annual inspection as required. Vessel is prohibited from operating until Coast Guard Annual Inspection or COI Renewal Inspection is completed.

Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station, conspicuously marked, and located at the midpoint of such vessel, or 100 feet from the engine, whichever is the more practical. The remote quick acting manual shutdown shall be installed on the engine so as to provide a quick and effective means of stopping the engine. Neither mechanical nor electric remote stops functioned at time of inspection. Prove proper operation of emergency stops to satisfaction of attending Marine Inspector

The cargo handling equipment shall be maintained by the tank vessel's personnel in accordance with the regulations in this subchapter, including the following: (b) Cargo pump relief valves shall be tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open. (c) Cargo pump pressure gage shall be tested at least once a year for accuracy. (d) The cargo discharge piping of all tank vessels shall be tested at least once each year for tightness, at the maximum working pressure No evidence provided of cargo piping, cargo pump relief valve, or cargo pump gauge being tested annually. Complete cargo equipment testing to the satisfaction of the attending Marine Inspector.

The operator of a vessel with a capacity of 250 or more barrels of oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154 shall provide transfer procedures that meet the requirements of this part and part 156 of this chapter for transferring - (a) To or from the vessel; and (b) From tank to tank within the vessel. No transfer procedures found on board vessel. Provide satisfactory transfer procedures to attending Marine Inspector.

All portable and semiportable extinguishers on board tank vessels shall be of an approved type. Both 40:B-C portable extinguishers require annual servicing to be of an approved type and missed annual servicing dates for 2021. Demonstrate portable extinguishers on board are of an approved type to the attending marine inspector.

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have -A means of draining or removing discharged oil or hazardous material from each container or enclosed deck area without discharging the oil or hazardous material into the water. Vessel is missing 4 plugs for containment. Prove to attending Marine Inspector that all containment drains have plugs.

(a) Electric installations on vessels must ensure: (1) Maintenance of services necessary for safety under normal and emergency conditions. (2) Protection of passengers, crew, other persons, and the vessel from electrical hazards. (3) Maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. Marine Inspector deemed wiring at cargo pump to be a significant electrical hazard to vessel personnel. Prove to attending Marine Inspector safe operation of electrical equipment and that system integrity is maintained.

A ship, except a ship of less than 26 feet in length, must have a placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station, stating the oil of discharge is prohibited.

A coaming, at least 4 inches high but not more than 8 inches high, enclosing the immediate area of the cargo hatches, loading manifolds, and transfer connections, that has a capacity, in all conditions of vessel list and trim to be encountered during the loading operation, of at least one-half barrel per hatch, manifold, and connection within the enclosed area. Missing 3 plugs in coaming

Shutoff valves. Shutoff valves shall be installed in the fuel supply lines, one as close to each tank as practicable, and one as close to each fuel pump as practicable. Valves shall be accessible at all times.

This section of the plan must contain information to assist the master in initiating action by the coastal State, local government, or other involved parties. This information must include guidance to assist the master with organizing a response to the incident, should a response not be organized by the shore authorities. Detailed information for specific areas may be included as appendices to the plan. See 33 CFR 151.26(b)(2) (Preamble) regarding a ship owner's responsibility to comply with individual state requirements for oil spill response. Per their Plan required notifcation to be located on board and was not present.

ISE not completed as required (31 Oct 2018). Make barge available for ISE upon arrival to Sector Hampton Roads zone. Cargo operations are not to be conducted until ISE is conducted and approved by Sector Hampton Roads USCG Marine Inspector.

Unable to verify if the cargo pump prime mover fuel hoses meet SAE J1942. Provide documentation that the hoses meet the standard or install hoses that meet SAE J1942, not to exceed 30 inches. In addition, hose fittings must comply with SAE J1475 and must be installed following the manufacturer's recommended practice.

Cargo pump relief valve had not been annually tested. Demonstrate proper operation of relief valve.

Cargo system piping pressure gauge had not been tested anually. Demonstrate accuracy of pressure gauge.

Provide MAWP rating of all cargo pipelines.

Repair damaged longitudinal weld in port side void #2, bay #2, #2 longitudinal from the bottom.
