In tankers, electrical equipment, cables, and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the organization. PSCO observed numerous lights with cracks in housing preventing them from being acceptable in hazardous locations.

The oily water seperator shall be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 ppm. The OWS bilge holding tank sample line was not providing effluent to the OCM. PSCO was unable to verify overboard discharge under 15 ppm.
