For tankers, the protection of the cargo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the fire safety systems code. PSCO observed a crack 50% of the way around the inert gas scrubber inlet flange.

The inert gas system shall be capable of inerting and maintaining the atmosphere in cargo tanks with an oxygen content of not more than 5% by volume. PSCO observed that the ship was unable to maintain IG below 5% using ships IG 02 analyzer.

Where the atmosphere in double-hull spaces cannot be reliably measured using flexible gas sampling hoses, such spaces shall be fitted with permanent gas sampling lines. PSCO observed fixed gas detection system not operational. System display panel stated several system failures.

In tankers, electrical equipment, cables, and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the organization. PSCO observed numerous lights with cracks in housing preventing them from being acceptable in hazardous locations.

The oily water seperator shall be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 ppm. The OWS bilge holding tank sample line was not providing effluent to the OCM. PSCO was unable to verify overboard discharge under 15 ppm.
