PSCO observed cable connections entering sounding gauges (float level gauge) on top of cargo tank 4, 3, and 2 were not properly connected. 74 SOLAS Cons VII / 13.1 93 IGC 10.1.4 40 a.c.

Gas detection systems provided in accordance with the requirements of 16.3.1 and 16.3.4 should comply with 13.6.2 and 13.6.4 through 13.6.8 as applicable; they should ativate the alarm at 30% of the lower flammable limit and shut down the master gas fuel valve referred to in 16.3.7 before the gas concentration reaches 60% of the lower flammable limit. Gas detection had an LFL reading in excess of 60% and master gas valve failed to shut down. 74SOLAS(14)VII/13.1 and IGC Code 16.3.10

Resolution: Received a valid International Certificate of Fitness with an equivalency issued IAW Section 1.4 of the 1993 IGC Code attesting to an equivalent level of safety regarding the Master Gas valves and the requirements of Section 16.3.7 and 16.3.8 of the code.
