The following functional requirements shall bemet: Means shall be provided to control leaks of flammable liquids; means shall be provided to limit the accumulation of flammable vapours. PSCO observed several pieces of machinery leaking oil. The HFO gate valve forward of the main engine has a catch pan to catch the leaking oil. No. 1 DG has excessive leaking pooling over drip pan. No. 3 DG lube oil auxiliary centrifugal pump had a bucket 1/3 full of lube oil from leaking pump. Port side bunker stationhad multiple soaked cardboard pieces & 5 gallon bucket of oil from leakage. Last bunkering was 2 weeks ago.
Condition: Improper/Lack of Maintenance

The arrangement onboard the ship for the extraction of samples from the 15 ppm bilge alarm should give a truly representative sample of the affluent with adequate pressure & flow. PSCO observed a valve in the sample extraction line. This valve is not included in the approved piping diagram provided by the ship's crew. PSCO believes the installed valve in the sample line can prevent the OCM from receiving a truly representative sample of the affluent.
