MARPOL 73/78 I/6.4.1 - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present convention to ensure that the ship in all respects will remain fit to proceed to sea without presenting an unreasonable threat of harm to the marine environment. Three pinhole leaks were discovered on the OWS system; one on the inlet pipe for the OWS bilge water heater and one each on two of the three OWS cylinders. (40ac)

The means of control of any fixed gas fire-extinguishing system shall be readily accessible and simple to operate and shall be grouped together in an few locations as possible at positions not likely to be cut off by fire in a protected space. At each location there shall be clear instructions relating to the operation of the system having reard to the safety of personnel. The manual isolation valve for the low pressure CO2 system was found in the closed position. Crew would not by able to engage CO2 for protected spaces from the remote location. The wrench to open the manual isolation valve was not present in the local space and took approximately one minute to retrieve. Each location did not have clear instructions that included the opening of the manual isolation valve.

The main steering gear shall be capable of putting the rudder over from 35degrees on one side to 35 degrees on the other side with the ship at its deepest seagoing draught and running ahead at maximum ahead service speed and, under the same conditions from 35 on either side to 30 degrees on the other side in not more than 28 seconds. Emergency steering unit does not put rudder over to 35 degrees port (vessel does not have full range of motion on port side)

Every oil fuel pipe which, if damaged would allow oil to escape from a storage, settling or daily service tank situated above the double bottom shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occuring in the space in which such tanks are situated. Oil Fuel tank valve for the emergency generator is inoperable.

After any survey of the ship has been completed, no change shall be made in the structure, equipment, systems, fittings, arrangements or materials covered by the survey. IOPP certificate certifies the separating/filtering equipment in accordance with MPEC.60 (33). Vessel has a MEPC. 107(48) system installed onboard.

Doors fitted in boudary bulkheads of machinery spaces of Catagory A shall be reasonably gas tight and self closing. Door from engine room to port side bunkering station was not self closing.
