Means shall be provided to control leaks of flammable liquids. PSCO observed active leakes on the main engine MGO supply line in multiple areas and return line from the purifier line. Catch basins were present under main engine cylinders #2 and #7 with accumulation of fuel oil. Pooled oil was observed in oil purifier room near heat exchangers and approximately 15 rags under M/E cylinders #1, 3, 4, 5, and 7. In addition, crew was observed actively painting over oil soaked lagging throughout E/R, reducing the ability to detect oil leaks on critical machinery systems.
