PSCO's observed that the cargo securing equipment on board the vessel was not permanently assigned to the ship. As per page 14 of the vessels Cargo Securing Manual, information about the type and specifics of the equipment is required to be on board. The vessel did not have documentation required as per page 14 of the manual dated 06/19/2010.

As per pages 12 and 14 of the Cargo Securing Manual, the manual should be updated by the Classification Society. PSCO's observed the Cargo Securing Manual was dated 06/19/2010 with no amendments of references in regards to devices in use.

02 - 74SOLAS(20)V/22.1.3 - 30abc - The horizontal field of vision from the conning position shall extend over an arc of not less than 225, that is from right ahead to not less that 22.5 abaft the beam on either side of the ship. PSCO's observed that cargo (wind turbine parts) obstructed all forward view from the vessel's conning station. The vessel installed a camera system with two cameras each having an individual arc of visibility of 87. PSCO's observed that the range of visibility did not meet 225.

Resolution: The top level of cargo was removed from the vessel, clearing any visual restrictions. - Downgraded to code 17 as a result of granted merit appeal by CVC.

PSCO's observed a totally obstructed field of vision from the conning station. The vessel's Safety Equipment Certificate did not reflect the physical arrangements of the vessel. No short-term certificate was issued.

08 - 74SOLAS(20)IX/3.1 ISM Code A/1.2.2 - 50abc - The company should ensure compliance with mandatory rules and regulations, assess all identified risks to its ships and personnel and establish appropriate safeguards, procedures, and instructions for key shipboard operations concerning the safety of personnel, ship and protection of the marine environment. PSCO's observed that cargo was stowed onboard the M/V JAY that caused contravention of the navigation bridge visibility regulationss. During the voyage to the US the Administration did not grant an exemotion of equivalency and issued a letter of dispensation on May 31, of which PSCO's observed the vessel did not foloow the procedures outlined in paragraph two of the letter. PSCO's observed that the cargo securing manual was not ammended for the procedures for the conditions on board, the company did not provide the vessel with the required information and procedures outlined in the cargo securing manual. An external audit is recommended.
