For tankers of 20,000 tonnes deadweight and upwards constructed on or after 1 July 2002 but before 1 January 2016, the protection of the argo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the Fire Safety Systems Code, as adopted by Resolution MSC.98/73, except that the Administration may accept other equivalent systems or arrangements as described in paragragh 5.5.4. The inert gas system referred to in Chapter II-2 of the Convention shall be designed, constructed, and tested to the satisfaction of the Administration. PSCO observed air leaking from windbox of IGG combustion chamber.

In tankers, electrical installations shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable. On deck explosion-proof lighting was corroded and cracked on amidship location, starboard side. Another explosion-proof lighting on deck, forward, bow area, starboard side, had several holes and corrosion on casing.

THE VARIOUS TASKS SHOULD BE DEFINED AND ASSIGNED TO QUALIFIED PERSONNEL. VESSEL MIS-SOUNDED TANK DURING BUNKER OPERATIONS, SPILLING AN UNKNOWN QUANTITY OF MDO OVER THE SIDE. RECOMMEND SMS AUDIT TO FOCUS ON BUNKERING PROCEDURES. - ISM (2018) A/7 - 40.A.B.C.
