Before the ship leaves port and at all times during the voyage. All life-saving appliances shall be in working order and ready for immediate use. All life raft painter lines were not properly secured to the deck. Starboard life raft was rigged with incorrect HRU. Corrected on the spot.

Each vessel vapor connection flange must have a permanently attached 0.5 diameter stud at least 1.0 inch long projection outward from the flange face. The stud must be located at the top of the flange, midway between the bolt holes, and in line with the bolt hole pattern. The portside forward and aft vapor connection flanges were missing studs.

Voyage Data Recorder (VDR) is not identifying alarms. Both Depth sounder and radar were shut down to find that VDR would not display alarm suggesting that the VDR is not recording properly. Technician was on board 16 April 2016 and reported issues to manufacturer (Danelec). Technician must identify problem and reason why VDR is not properly reading correctly.
