Each operation shall be fully recorded without delay in the ORM part 1. ORB Part 1 recorded an OWS operation of 15.0 cubic meters discharged through 15 ppm oil filtering equipment in 4.5 hours. OCM has no recording of any overboard oil discharge through 15 ppm equipment on 09 APRIL 2019. Sounding logs of the bilge holding tank reflect a 19.1 cubic meter unaccounted decrease in the tank from 01 APIL 2019 to 03 April 2019. MARPOL 73/78 ANNEX I 17.4 17 abc
Condition: Improper/Lack of Maintenance
Action required: 17 - Rectify deficiencies prior to departure
Resolved 26 July 2019
Resolution: DNV GL attended the vessel for repair and calibration of OCM recording device. New SCAN Disk Memory card replaced, electrical wire connections made secure and tight. See deficiency documents for class report.
