Measures shall be taken to the satisfaction of the administration to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. PSCO found the oil mist detectors to be inoperable and in need of maintenance.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. PSCO observed the main engine cylinders #1,24,5,6 have broken sight glasses. PSCO observed a broken sight glass for the heavy fuel oil line in the purifier room.

In tankers, electrical equipment, cables and wiring shall not be installed in hazaardous locations unless it conforms with standards not inferior to those acceptable to the organization, inspector identified 01 explosion proof fixture with 01 broken tombstone on live voltageside of LED bulb with non-voltage side disconnected; one explosion proof fixture with 01 securing cups missing; and 03 bonding straps disconnected/ broken on cargo deck.
