Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed an excessive amount of oil spraying out of exhaust vent. Lifeboat was unable to go astern. 74 SOLAS (2009) III/20.2

PSCO observed exhaust gas from free fall lifeboat entering the engine room and causing the fire alarm to sound. Neither Captain nor crew responded to smoke or alarm. 74 SOLAS (2009) III/18.2.1
