After any survey on the ship under regulation 7,8,9 or 10 has been completed, no change shall be made in the structural arrangement, machinery, equipment and other items covered by the survey, without the satisfaction of the Administration. Flame screen on number 4 port and 6 stbd cargo tank P/V valves has larger holes than permitted under flame screen mesh size requirements. 74SOLAS(20) I/11(b)

Every ship 400 gross tonnage and above shall carry a garbage management plan that the crew shall follow. This plan shall provided written procedures for collecting, storing, processing and disposing of garbage including the use of equipment onboard. The garbage management plan is not being followed, the PSCO observed plastics in the incinerator. The garbage management plan onboard specifically states that incineration of all plastics is prohibited.

Resolution: PSCO examined the garbage record book and observed no other instances of incineration of plastics onboard, as well as reviewed garbage management plan to ensure that the burning of plastics is forbidden
