Manual brakes shall be so arranged that the brake is always applied unless the operator, either on
deck or in the survival craft or rescue boat, holds the brake control in the "off" position. While lowering lifeboat #3, it was found that the manual brake could only be applied if it was brought down into place, as the operator could not slow or stop the descending from inside the lifeboat. Additionally, lifeboat #3 has a winch brake that will not lower on its own, as there is some kind of binding in the lever or wire remote. Make repairs to demonstrate proper operation of braking systems for lifeboat #3 to the satisfaction of Class (ABS) prior to departure.

A launching appliance shall not depend on any means other than gravity or stored mechanical power which is independent of the ship's power supplies to launch the survival craft or rescue boat it serves in the fully loaded and equipped condition and also in the light condition. During inspection, it was found that the pendant for lifeboat #1 had a “stopper” in a way that did not allow the operator of the lifeboat to deploy it from the davits. Make repairs to demonstrate proper lowering operation of lifeboat #1 to the satisfaction of Class (ABS) prior to departure.

With the exception of the secondary means of launching for free-fall lifeboats, each launching appliance shall be so arranged that the fully equipped survival craft or rescue boat it serves can be safely launched against unfavorable conditions of trim of up 10° and list of up to 20°either way. While launching lifeboat #1 during inspection, the lifeboat made contact with side of ship, which destroyed two of the aft removable fenders, but did not appear to affect the hull of the lifeboat. Fenders need to be replaced to protect the lifeboat from another impact if needed. Replace to satisfaction of Class (ABS).

The normal equipment of a fast rescue boat shall include a VHF radiocommunication set which is hands-free and watertight. During annual inspection, the vessel was not able to lower port side lifeboats (#2 and #4) due to being pier side. Additionally, there were deficiencies found on the launching arrangements for the starboard side lifeboats (#1 and #3). Make available the port side lifeboats for lowering and maneuvering for witness by USCG and/or ABS (if USCG cannot attend).

The lifebuoys shall be fitted with retro-reflective material (tape) with a minimum of 5 cm (2 inches) width around the lifebuoy at four locations spaced equally around the circumference of the lifebuoy. Retro-reflective material shall be Type II material that is approved under 46 CFR 164.018. Ring bouys throought main deck were taped with DOT C2 reflective tape. Replace all DOT C2 tape with SOLAS approved in accordance with 46 CFR 164.018 and IMO Resolutio A.658(16).

During a routine launch, the port rescue boat engine died. Vessel had to launch #4 lifeboat to recover the rescue boat. Cause of engine failure is believed to be a clogged fuel line between the fuel tank and strainer. Flushed fuel line and are awaiting op test at the next opportunity in the port.USCG/MSC MOA of
02 JUL 2018 allows MSC ships with one operational rescue boat to sail provided a repair plan for the inoperative rescue boat is submitted to USCG.
