Replace fireboy bottles and provide servicing report. Port and stbd fireboy bottles are corroded and bloating. Demonstrate operation of system for MI's prior to operating. 46 CFR 176.810

Fuel leak observed in the bilge of the port hull fuel tank. Remove fuel from the bilge and identify source of the fuel leak. Submit fuel tank repair proposal to USCG for review and approval prior to conducting repairs. After fuel tank repairs are completed, conduct satisfactory hydrostatic pressure tests in the presence of USCG marine inspectors IAW 46 CFR 182.440(c)(1); hydrostatically tested to, and must withstand, a pressure of 35 kPa (5 psig) or 11/2 times the maximum pressure head to which they may be subjected in service, whichever is greater. A standpipe of 11.5 feet in height attached to the tank may be filled with water to accomplish this 5 psig test. 46 CFR 176.700

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Lazarette, machinery space, main deck non watertight hatch. Attending MI's noted that of the 6 spaces that require high water bilge alarms only the Starboard Engine compartment was operational. Prior to operations with passengers all high water alarms shall be operational. 46 CFR 182.530 (a)

Attending MI's noted that the support bracket for the port forward handrail was cracked. Make needed weld repairs to the cracked aluminum. 46 CFR 177.900

Unless otherwise stated in this section, life jackets must be stored in convenient places distributed throughout accommodation spaces. Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Life jackets were stowed in a manner that does not separate child and adult sizes. Attending Mis noted that life Jackets were stowed in a partially submerged environment. Vessel must separate sizes and provide a more suitable manner for stowage to prevent being stored in the bilge water. 46CFR180.78(a)(5)

Each life jacket carried on board the vessel must be marked in accordance with § 185.604 of this chapter. Each life jacket must be marked with Type I retro reflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Attending Mis noted that the retro reflective material on over half the required life jackets were no longer serviceable. Vessel must repair or replace the worn retro reflective material on all life jackets.46CFR 185.604(h)

All other vessels must undergo an inspection for certification as specified in 176.404 and annual inspection as specified in paragraph (b)(1) of this section. (1) Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel was not subjected to an Annual COI inspection within 3 months after the anniversary date.

Pre-engineered fixed firefighting system port and starboard engine visual indicators at the operating station failed to operate during testing of shutdowns.

The crew was unable to prove proper operation of the port and starboard bilge suction systems during onboard testing.

The plastic panel at the operating station that houses the engine RPM, temperature, and oil pressure gauges is cracked and not providing protection from water intrusion.

Re-stencil or replace life jacket stowage marking placards.

Re-stencil or replace fire extinguisher location marking placards.

Replace jib sail and port/starboard blocks for main sail running rigging located above main deck cover in accordance with sail rigging report prior to using jib.

24" lifebuoy taken out of service due to UV damage. Replace in kind.

Provide protective guards for port and starboard main engine exposed pulleys.

Prove proper operation of fire pump switch.

Make bowman hatches operational.

Repair tear in main sail.

Replace frayed strapping on port and starboard trampolines.

Provide 01 additional 40-B:C fire extinguisher outside engine room.

Vessel has not implemented or recorded weekly and quarterly rigging inspection/maintenance in accordance with the company's Rigging Inspection and Maintenance Plan.
