PSCO observed from the steering gear room when conducting the steering gear test on the #2 steering pump, hunting or oscillation through the hydraulic system when going from midship to port. The construction of doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated, this is being determined in accordance with the fire test procedures code. PSCO observed missing or unapproved tire gaskets on all A60 self closing doors located in the main ladder well leading to the bridge, all escape doors in the machinery space and doors leading to the steering gear room. Code: 17ac 74SOLAS(09) II-2 Reg 9.4.1.1.2

After any survey of the ship under regulations 7,8,9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the administration. PSCO observed temorary repairs on GS pump eductor line and bottom of cascade tank pipe as noted in vessel's flag inspection report. Code: 17bc 74 SOLAS (24) I Reg 11(b)

For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO observed wasted and oil soaked lagging throughout the machinery spaces as noted in the vessel's flag inspection report. Code 17bc 74 SOLAS (09) II-2 Reg 4.1.1

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with additional requirements which may be established by the company. Due to the objective evidence in the .deficiencies noted below, the vessel is not being maintained and not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel's SMS under the ISM code. An external audit conducted by the Flag or RO is required before the ship will be released from its detention to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to USCG prior to departure from port.

“Fire hoses” shall, together with any necessary fittings and tools, be kept ready for use in conspicuous positions near the water service hydrants or connections. PSCO observed (2) fire hoses with holes and multiple fire hose quick connects severely corroded.

Where a fixed gas fire extinguishing system is used, openings which may admit air to, or allow gas to escape from, a protected space shall be capable of being closed from outside the protected space. PSCO observed drain valves deteriorated to the point of inoperability, missing entirely, or missing fire caps on all 5 cargo holds.

Bulk carriers shall comply with the maintenance requirements provided in regulation II-1/3-1 and standards for owner’s inspection and maintenance of bulk carrier hatch covers, adopted by the organization by resolution MSC.169(79). Cargo hatch ventilation openings were found with covers corroded, rough, uneven, and with hinges deteriorated and thinned down or otherwise incapable of being sealed.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed CO2 discharge piping for cargo holds heavily corroded.

Maintenance, testing, and inspections shall be carried out based on the guidelines developed by the organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. PSCO observed last annual service report for water mist system completed in 2021.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed the following conditions that require rectification to include but not limited to rescue boat davit wire cable for accumulator lever improperly installed, limit switch stop on the rescue boat davit heavily corroded and broken, and safety stop flywheel heavily corroded.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed the outboard engine fuel line disconnected from over pressurization.

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration. PSCO observed improper cooling water pipe repairs on all three generators.

The machinery, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to person on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended. PSCO observed missing machinery guards on all three generators and two exhaust leaks in the generator exhaust piping.

The purpose of this regulation is to contain a fire in the space of origin. For this purpose, the following functional requirements shall be met: thermal insulation of boundaries shall have due regard to the fire risk of the space and adjacent spaces. PSCO observed multiple locations of degraded structural fire protection in the engine room and escape trunk in the engine room.

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed the following conditions that require rectification to include but not limited to Majority of outdoor stuff tubes and electrical grommets either wasted or missing on house bulkheads and cargo hold outlets, and MF/HF High-Voltage antenna installation on flying bridge not grounded.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed the following conditions that require rectification to include but not limited to multiple light fixtures improperly mounted with loose electrical wiring, unapproved junction box on cargo hold mounted to the CO2 pipe, and electrical cable to port pilot boarding area not safely ran and laying on the deck.

Satisfactory means (in the form of guard rails, lifelines, gangways or underdeck passages, etc.) shall be provided for the protection of the crew in getting to and from their quarters, the machinery space and all other parts used in the necessary work of the ship. PSCO observed but not limited to; guard rails wasted/detached in multiple spots, gangway railing mounts heavily corroded, and cargo hold guard rail access ladder heavily corroded.

Ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society which is recognized by the administration. PSCO observed active hydraulic oil leak on forward mooring equipment in bosun’s stores.

From machinery spaces other than those of category A, two escape routes shall be provided except that a single escape route may be accepted for spaces that are entered only occasionally and for spaces where the maximum travel distance to the door is 5 m or less. PSCO observed watertight door leading from escape ladder to the fantail was inoperable.

Oil fuel pipes and their valves and fittings shall be of steel or other approved material. PSCO observed improper fuel line section on number 2 generator.

Bulk carriers shall comply with the maintenance requirements provided in regulation II-1/3-1 and standards for owner’s inspection and maintenance of bulk carrier hatch covers, adopted by the organization by resolution MSC.169(79) . PSCO observed hatch cover cleats with dry rotted rubber washers.

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the administration in accordance with the provisions of regulation XI-1/1, or with applicable national standards of the administration which provide an equivalent level of safety. PSCO observed sounding tube caps not properly installed.

The condition of the ship and it’s equipment shall be maintained to conform with the provisions of the present convention to ensure that the ship in all respects will remain fit to proceed to sea without presenting an unreasonable threat of harm to the marine environment. PSCO observed hydraulic oil spilled on deck in vicinity of number 2 cargo hold and oil containment cofferdams underneath hydraulic piping deteriorated or otherwise incapable of being sealed in vicinity of cargo holds two and four.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed multiple clogged ventilation supply ducts in the engine room.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed port side bridge wing life buoy and free fall lifeboat with severely degraded retro reflective tape.

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. They shall also be permanently marked on the sides of the ships to the satisfaction of the administration. The marks shall be plainly visible. PSCO observed load line markings are not contrasting in color to background.

An adult life jacket shall be so constructed that the method of securing the life jacket to the wearer has quick and positive means of closure to that do not require tying of knots. Lifejackets on board can only be secured by tying knots. 74SOLAS(14)III/7.2.1 LSA CODE 2.2.1.5.4

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Documented A15 boundary had holes drilled into it. 74SOLAS(14)1/11(a)

Provisions shall be made to facilitate cleaning, inspection and maintenance of propulsion and auxiliary machinery. Large amounts of oil were noted leaking from machinery throughout engine room. 74SOLAS(14)II-1/27.7

Cylinders mst always be secured and when not in use they must be stowed in a rack in an upright position with the valve protection cap in place. Oxygen and acetylene bottles in the lockers are not secured. 46CFR147.60(b)

The means for securing and maintaining weathertightness shall be to the satisfaction of the administration. Weathertight doors were found to have wasted gaskets. Light is visible from inside. ICLL(88)16.4

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Crew was unfamiliar with SCBAs and other equipment. 74SOLAS(14)III/19.2.1
