Electrical installations shall be such that the safety of the crew and ship from electrical hazards. PSCO noted lighting in pump room grounding material and end caps were missing and/or loose that has the potential to introduce the risk of fire or explosion in the event of an electrical fault.

Resolution: New ground straps were installed and end caps were properly secured to prevent risk of fire or explosion.

Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of an electrical fault in such areas, special precautions against such risks shall be taken. PSCO observed cargo pump bearing temperature thermocouples' wiring connections to pump casing (upper and lower) are not installed to retain the original electrical safety properties.

Resolution: All wiring connections were properly sealed by ships crew with new packing.

Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. PSCO observed the starboard lifeboat was unable to start.

Resolved 8 December 2021
Resolution: SHIPS CREW FOUND DEBRIS IN THE FUEL LINE. THEY BLEW OUT LINE TO CLEAR DEBRIS AND RAN ENGINE WITH NO ISSUE.
