A lifeboat with a self-contained air supoort system shall be so arraned that te air in the lifeboat reamins safe and breathable for a period of not less than 10 min. The starboard lifeboats breathing air bottles are leaking with all valves closed. SOLAS 20 III/31.1.6 LSA CODE 4.8

Resolution: The leakage was identified at the manifold connection of the breathing air bottle. The connection was repaired and the air bottle was recharged to 200 BAR using the BA compressor.A bubble test was conducted confirming that no further leakage was present.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed vessel had added unapproved electrical wiring to the crew laundry room which included splices which were made to allow operation to the freezer. 74SOLAS(04)II-1/45.1.3 10c

The company and the ship shall comply with the requirements of the international safety management code. PSCO observed a crew member enter the lifeboat without a personal gas meter or ventilating the lifeboat. The PSCO's gas meter began to alarm for CO gas. The lifeboat entry guidelines in PRW-09 if the vessel's ISM were not followed.
