Ullage openings shall not be used for pressure equalization. They shall be provided with self-closing and tightly sealing covers. PSCO observed product (Naphtha) on deck, coming from main cargo tank (Port Tanks: 2, 3, 4, 5, 6 & Stbd Tanks: 4, 5, 6). Gasket on ullage openings is worn. Vessel cargo tanks levels are below 95%, and according to crew, seepage of cargo occurred due to heavy seas encountered during voyage. CODE 17 a c - 74 SOLAS 14 (CONS.) II-2/4.5.3.3

Personnel responsible for the operation of our incinerator installed in accordance with the requirements of paragraph 6.1 of this regulation shall be trained to implement the guidance provided in the manufacturers operating manual as required by paragraph 7 of this regulation. Incinerator door is fitted with temporary repair.
