When the emergency generator is automatically started it should be automatically connected to the emergency switchboard. When conducting ESD start of the emergency generator the generator started in under 10 seconds but would not connect to emergency switchboard due to low current.

In approving structural fire protection details the Administration should have regard to the risk of heat transmission at intersections and terminal points of required therm barriers. The vessel had numerous locations identifed as class A boundaries on the approved fire control plan that were missing SFP insulation, self closing doors would not latch or were missing the closing mechanism and cable MCT were missing.

The power available should be sufficient to supply all those services essential for safety in an emergency. For a period of 18 hours, one of the fire pumps, if dependent upon the emergency generator for its source of power. The sea raw water pump that runs off of the emergency generator was inoperable and the
vessel did not have enough fresh water on board to meet the 18 hour requirement.
