The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fir to proceed to sea without danger to the ship or persons on board. While under pilotage inbound to Oil Dock #4, the vessel experienced repeated alarms and loss of tachometer indication when dead slow astern was ordered; master failed to report this propulsion abnormality to the Coast Guard, even after being advised by the pilot. Make proper repairs and provide class report addressing the causative factors and corrective measures.
