To discharge ballast water into the waters of the United States, the master, owner, operator, agent or person in charge of a vessel subject to 151.2025 of this subpart must either ensure that the ballast water meets the ballast water discharge standard in 151.2030(a), use an AMS as described in 151.2025(a)(3), or ballast with water from a U.S. public water system, according to the schedule in paragraph (b) of this section. Subject vessel, of keel laid date 31DEC2009 and ballast water capacity of 41121 m3, is past its compliance date per the implementation schedule in 33 CFR 151.2035 (b). Currently, vessel’s installed BWTS is inoperative. Vessel is not authorized to discharge untreated ballast into the waters of the U.S. without prior written authorization from the local Captain of the Port under the provisions of 33 CFR 151.2040 (b).

Doors required to be self-closing shall not be fitted with hold-back hooks. Observed 02 class A self closing doors fitted with hold back devices. 74 SOLAS(09)II-2/9.4.2.2

In Tankers cargo pumps, ballast pumps, and stripping pumps, installed in pump rooms shall be fitted with temperature sensing devices for bulkhead shaft glands, bearings and pump casings. The bearing temperature sensor on the #2 COP was fully retracted from its housing. 74 SOLAS (09) II-2/4.5.10.1.1

Each item of remote operating or indicating equipment, such as a remotely operated valve, tank level alarm, or emergency shutdown device, must perform its intended function. PSCO observed that the portside ESD did not actuate the closure of the portside #3 cargo line valve. Vessel can only close valve from CCR and STBD side ESD. 33CFR156.170(c)(5)

Steam line passing through the purifier space was found to be leaking. The pipe in refernce is the steam pipe to the HFO settling tank.
