An immersion suit of an appropriate size, complying with the requirements of section 2.3 of the Code shall be provided for every person on board the ship. One crew member needs extra large immersion suit. Provide appropriately sized immersion suit prior to operating above 32N or below 32S. SOLAS 74am (20ConsEd) III/32.3.2

Where they pass through accommodation spaces or spaces containing combustible materials, the exhaust ducts from galley ranges shall be constructed of "A" class divisions. Each exhaust duct shall be fitted with a fire damper located in the lower end of the duct. During fire drill, galley fire damper #3 failed to close. Crew made repairs. SOLAS 74am (09ConsEd) II-2/9.7.5.2.1

In the case of a lifeboat arranged for free-fall launching, at least once every three months during an abandon ship drill the lifeboat shall be free-fall launched with only the required operating crew on board, or lowered into the water by means of the secondary means of launching with or without the operating crew on board. In both cases the lifeboat shall thereafter be maneuvered in the water by the operating crew. Vessel’s PMS only indicates simulated launch every 6-months. Last recorded maneuvering of lifeboat in the water was Feb 2025. Captain requested update to company PMS for launch/in-water operation every three months and crew launched lifeboat during USCG inspection. SOLAS 74am (20ConsEd) III/19.3.4.4

Air pipes to ballast tanks that extend above freeboard or superstructure deck shall have satisfactory means, permanently attached shall be provided for closing the openings of the air pipes. Ballast tank vent head closures (floating ball type) found non-operational due to rust. Crew conducted maintenance and Captain added monthly safety inspection to PMS. ICLL 66 Reg. 20

Deep-fat cooking equipment installed in enclosed spaces or on open decks shall be fitted with the following: 1) an automatic or manual fire-extinguishing system tested to an international standard acceptable to the Organization; 2) a primary and backup thermostat with an alarm to alert the operator in the event of failure of either thermostat; 3) arrangements for automatically shutting off the electrical power upon activation of the fire-extinguishing system; 4) an alarm for indicating operation of the fire-extinguishing system in the galley where the equipment is installed; and 5) controls for manual operation of the fire-extinguishing system which are clearly labelled for ready use by the crew. During inspection, a new deep-fat fryer was found installed in vessel’s galley. Equipment does not meet above standards. The crew removed deep-fat fryer and cabling from galley. Whenever a vessel is deprived of the service of a member of its complement, and the master or person in charge is unable to find appropriate credentialed personnel to man the vessel […] a report of sailing short must be filed with the OCMI having cognizance for inspection in the area in which the vessel is operating, or the OCMI within whose jurisdiction the voyage is completed. It shall be the duty of the owner, master, or person in charge to require and have performed at least once in every twelve months the tests and inspections of all hand portable fire extinguishers, semi-portable fire extinguishing systems, and fixed fire extinguishing systems on board. Vessel has been unable to schedule required tests and inspections by competent authority, but shall do so as soon as reasonably possible, but no later than 30 days from the date of expiry of current certificates.

All inlets and discharges in the shell plating shall be fitted with efficient and accessible arrangements for preventing the accidental admission of water into the ship. The OWS overboard discharge skin valve is leaking;replace in kind during upcoming drydock in June 2019.

All ships which undergo repairs, alterations, modifications and outfitting related thereto shall continue to comply with at least the requirements previously applicable to these ships. Three grey water pipes in the steering gear room, one grey water pipe in the lower deck of the engine room, and a hose fitting on the tank top of the MSD were discovered with soft patch repairs; Conduct satisfactory permanent repairs during upcoming drydock in June 2019.

Proper inspection of the vessel's cargo area was not able to be completed by the attending USCG inspectors due to the holds and deck being fully loaded with cargo. Make all cargo holds and cargo deck available for inspection during upcoming drydock exam.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection including fire-resisting divisions, and protection of openings and penetrations in these divisions. Dampeners AO2, AO4, AO7, and AE32 are incapable of proper closure. Conduct satisfactory permanent repairs.

Numerous weatherdeck ventilation closures were found to be incapable of closure due to painting over the hardware. Conduct repairs and ensure that all hatches are capable of being closed.

To discharge ballast water into the waters of the United States, the master, owner, operator, agent, or person in charge of a vessel subject to 151.1510 must ensure the ballast water meets the ballast water discharge standard as defined in 151.511(a). Vessel must install CG approved BWMS prior to entry into US waters.

Posters or signs shall be provided on or in the vicinity of survival craft and their launching controls and shall use symbols in accordance with the recommendations of the Organization. Ensure that all required equipment is present and labeled in accordance with the vessel's approved LSA plan.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Two horizontal vibration mounts on the main engine were discovered to have factures in the base plate welds and base metal; Conduct satisfactory permanent repairs during upcoming drydock in June 2019.

The operation of a marine diesel engine that is installed on a ship constructed on or after 1 January 2000 and prior to 1 January 2011 is prohibited, except when the emission of nitrogen oxides (calculated as the total weighted emission of NO2) from the engine is within the following limits, where n = rated engine speed (crankshaft revolutions per minute): 17.0 g/kWh when n is less than 130 rpm. Vessel must have EIAPP certificate issued by the US EPA for each marine diesel engine with a power output of more than 130 kW installed EPA must approve each technical file as evidence by issuance of an EIAPP certificates. Vessel must provide EPA issued EIAPP certificates for all applicable marine diesel engines as proof of emissions compliance for IAPP certificate.

Where main or auxiliary machinery including pressure vessels or any parts of such machinery are subject to internal pressure and may be subject to dangerous overpressure, means shall be provided where practicable to protect against such excessive pressure. The main engine and aux generator start air receiver relief valves were set and relieved above the maximum allowable working pressure (MAWP); Renew start air relief valves with ones that are set to no more than MAWP and sized to prevent the pressure in the vessel from rising 10% or .21 bar whichever is greater.

In order to achieve the fire safety objectives set out in paragraph 1, the following functional requirements are embodied in the regulations of this chapter as appropriate: .1 division of the ship into main vertical and horizontal zones by thermal and structural boundaries; numerous fire rated Marine Cable Transits (MCT) have been damaged due to new equipment installations. Conduct repairs to damaged MCT’s.

Each lifeboat shall be provided with an appliance which is capable of launching and recovering the lifeboat. The free fall lifeboat's falls are parted making it impossible to safely recover the lifeboat; replace falls.

Systems and equipment used to meet the requirements of this regulation shall conform to performance standards and functional requirements not inferior to those adopted by the Organization. Any shipboard equipment shall be of a type approved by the Administration. Vessel must complete LRIT installation and provide documentation of conformance test.

All automatically or remotely controlled or monitored vital systems addressed by part 62 must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. The test procedures must be submitted to the Coast Guard for approval. Vessel has requested to operate with periodically unattended machinery space (PUMS) but does not have CG approved periodic safety test procedures(PSTP). Vessel has been operating with ABS class ACCU designation under previous flag. Interim PUMS acceptance is granted until next credit dry dock scheduled for June 2019. Required manning will be that of a fully manned engine room, as indicated on COI, until the PSTP are submitted and approved by the Marine Safety Center or by and authorized ACS.

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500 l and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Quick closing F/O valves for auxiliary diesel generator NO 1, NO 2, and NO3 did not function due to a hole in the control air line actuating the valves. Crop and renew damaged section of air tubing and re test to the satisfaction of the attending MI.
