Efficient guard rails or bulwarks shall be fitted to all exposed parts of the freeboard and superstructure decks. Wire ropes should be accepted in lieu of guard rails in special circumstances and then only in limited lengths. Lengths of chain should be accepted in lieu of guard rails if they are fitted between two fixed stanchions and/or bulwarks. There were several sections of guard rail between stanchions that were regular rope; replace with acceptable materials. Posters or signs shall be provided on or in the vicinity of survival craft and their launching controls and shall illustrate the purpose of controls and the procedures for operating the appliance and give relevant instructions or warnings. Every lifeboat and liferaft was missing the survival craft launching procedures. Cite: SOLAS (2004) III/Reg 9.2.1

At least one radar transponder shall be carried on each side of every cargo ship of 500 gross tonnage and upwards. When testing AIS-SART functionality, test did not indicate on AIS as outlined in procedure. Technician identified SART function was not enabled on AIS. Cite: SOLAS (2004) III/6.2.2

Identified the following electrical hazards: -Cable glands were broken/missing/improperly installed in stbd tunnel WTD indicator light/sensor junction boxes, lights on stbd main deck, local winch control junction boxes on forecastle, improper stopping plugs in control panel penetrations in port/stbd tunnels and stbd deck -Light covers were not properly installed on stern mooring station lights. -Improper wiring of light and outlet on stbd side main deck. -Missing switch toggle for light in battery room -Unsupported light fixtures in port-stbd athwartship’s tunnel at ladder to forecastle and port side light adjacent to liferaft. -port side tunnel cargo fan control panel with extension cord improperly installed through panel box. -Exterior light inboard of paint locker improperly installed cable glands, wiring, and stopping plug. -Electrical cable/plug for rescue boat was no longer serviceable. Administrations shall take account of the danger posed by the fatigue of seafarers especially those whose duties involve the safe and secure operation of a ship. Administrations shall require that records of daily hours of rest of seafarers be maintained in a standardized format to allow monitoring and verification of compliance with the provisions of this section. Observed work rest hour tracker program "ISF WATERKEEPER 3.52.2" does not reflect work hours completed during overtime. The work rest tracker fails to show work hours as observed within overtime log. As a result fatigue factors cannot be adequately accounted for. In accordance with vessel's "Maersk Line, Limited's Guidance on STCW Rest Hour Management" Section 3.4, compliance with relevant rest hour regulations is mandatory. Request an internal SMS audit regarding tracking of work and rest hours to include tracking of overtime work hours in conformity with STCW.

Received a report from Pilots of an unsafe pilot ladder arrangement presenting a hazard to the port and Maritime Transportation Members. Provide a causal-analysis report identifying the contributing equipment, training and maintenance factors that led to the unsafe pilot embarkation arrangement, as well as a report from the vessel’s flag administration certifying that necessary corrective actions have been satisfactorily completed and the vessel is equipped with compliant pilot embarkation arrangements

Provide a causal-analysis report to the Coast Guard identifying the contributing equipment, training and maintenance factors that led to the unsafe pilot embarkation arrangement.

All automatically or remotely controlled or monitored vital systems addressed by Part 62 of this Subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Vessel did not have approved procedures on the vessel.

The inspections mentioned in 10.2 as well as the measures referred to in 10.3 [maintenance of equipment] should be integrated into the ship's operational maintenance routine. Vessel had approximately 70 overdue maintenance items in the engineering department. Recommend ISM external verification focusing on engineering department.

Periodic safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must must include fire detection and extinguishing, flooding safety, propulsion, manuervering, electrical power generation and distribution, and emergency internal communications. Approved testing procedures have not been completed as required by company policy. Some items have not been checked in two years.

The machinery boilers and other pressure vessels, associated piping systems and fittings, shall be of a design and construction adequate for the service for which they are intended. The habdrail mount for the #4 generator was broken and no longer serviceable.

Level sensor on OWS is not working properly and the 3 way valve does not switch over as it is intended.

During testing of the vessel's EDG automation, crew demonstrated that testing of automation procedures did not match the approved periodic test procedures. PSTP shall be amended to match correct testing procedures.

There shall be adequate means to alert the bridge regarding machinery operations from the steering gear room. Vessel's bridge sound powered phone did not alert the bridge crew when crew from the steering gear room attempted to make contact with the bridge. Verify that the audible (ringer) and visual indicators work properly. 74 SOLAS (14) V/19.2.1.9

1. In accordance with the issued letter of deviation 051-17 dated 07 AUG 2017, please provide technical report attesting to the proper operation of the automatic radar plotting aid (ARPA).

The vessel S band 3 GHz radar is not powering up; therefore, the vessel only has one 9GHz radar. Verfication of properly operating S band 3GHz radar to be completed to the satisfcation of the attending CG Marine Inpsector or LR Class surveyor. SOLAS V/19.2.7.1

Provide EIAPP certificate for main engine and auxiliaries. In the interim, the vessel is authorized to sail on a short-term IAPP issued by the ACS which is to be accompanied by the EIAPP supplements issued by Lloyds Register on behalf of Singapore. Verify EIAPPs to the satisfaction of CG Marine Inspector or LR Class surveyor. MARPOL VI

An immersion suit which also complies w/ the requriements of life-jackets may be classified as a life-jacket. It shall be fitted with a light and the whistle complyin gwith the requriements of life-jackets. Immersion suits are not outfitted w/ lights and are classified as life-jackets. Immersion suits to be outfitted w/ approved lights to the satisfaction of CG Marine Insp or LR class surveyor.
