Vessel reported the rescue boat engine starter was discovered to be inoperable during routine checks. New part is on order and will arrive at vessel's next port of call in Antwerp, Belgium on 11Sep2025. Rectification is contingent upon successful installation and in person survey by class.

Instructions for on-board maintenance of life-saving appliances shall be easily understood, illustrated wherever possible, and, as appropriate shall include for each appliance: log for records and maintenance. Rescue boat was found to have unauthorized repairs done to the underside of the rescue boat where it rests in the cradle. There are no records of the when the repair was conducted or approval from Class. 74SOLAS(2020) III/36.7

Fire protection appliances shall be maintained ready for use, a fire extinguisher in the reefer deck workshop was blocked and not ready for immediate use. The area was immediately cleared.

A duplicate set of fire control plans or a booklet containing such plans shall be permanently stored in a prominently marked weather-tight enclosure outside the deck-house for the assistance of shore-side fire-fighting personnel. The fire plan in the enclosures were revision #04 when there is a newer revision on-board. Ensure that all fire control plans on-board are of the most current revision.

The machinery and associated fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts. The turbo return valves are leaking oil on the #1 & #3 generators make proper repairs.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Vessel is missing High Voltage safety equipment. Provide the proper high voltage personal equipment.

The company and the ship shall comply with the requirements of the International Safety Management Code. As per the vessel's safety management system, there are over 50 maintenance items that are overdue. Maintenance needs to be completed to the satisfaction of the Recognized Organization, American Bureau of Shipping.

Measures shall be taken to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. Found that the on-board Periodic Safety Test Procures not being fully tested nor is there any recording/log of when the tests when conducted. To the satisfaction of the Recognized Organization Lloyd’s Register of Shipping, ensure the testing and recording are done in a way to ensure that the there is evidence that the tests are being conducted.

Whenever a vessel is deprived of the service of a member of its complement, and the master or person in charge is unable to find appropriate credentialed personnel to man the vessel MAERSK COLUMBUS a report of sailing short must be filed with the OCMI having cognizance for inspection in the area in which the vessel is operating, or the OCMI within whose jurisdiction the voyage is completed. Vessel master/rep made sailing short notification for the position of Able Seaman from Newark, NJ to Algeciras, Spain. 46 CFR 15.725

Whenever defect is discovered, the master or owner of the ship shall report at the earliest opportunity to the Administration or Recognized Organization (RO). Back in 2020 the vessel reported to the operator of a clutch issue (not aligned) to the port side anchor windless, but never reported it to the Administration or RO. In addition the vessel just came out of dry-dock in June 2022 with the clutch not aligned, there were also several anchor chain links and chain stopper pins, that were known to be bent but not repair or replaced while in the dry-dock.

Falls used in launching shall be inspected periodically with special regard for areas passing through sheaves, and renewed when necessary due to deterioration of the falls or at intervals of not more than 5 years, whichever is the earlier. Falls for both port and starboard lifeboats were due to be replaced by 03JAN2022. Replace both sets of falls by the end of the next scheduled drydock.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Several non-metallic expansion joints on the #1, #2, and #3 service diesel generators were found cracking.

Load Line and draft marks: The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. The marks shall be plainly visible and, if necessary, special arrangements shall be made for this purpose. Load line and draft marks were not visible on the port side of the vessel.

Hoses specified in this chapter as "fire hoses" shall, together with any necessary fittings and tools, be kept ready for use in conspicuous positions near the water service hydrants or connections. One hose on starboard side main deck had leaks on both ends, replace with new hose.

Waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. Large waste receptacle on the bridge is made of combustible material.

It shall be the duty of the owner, master, or person in charge to require and have performed as least once in every twelve months the tests and inspections of all hand portable fire extinguishers, semiportable fire extinguishing systems, and fixed fire extinguishing systems on board. Portable fire extinguishers were required to be serviced by 03 February 2022 and were not.

Every vessel equipped with a radio installation on an international voyage must have a Cargo Ship Safety Radio Certificate. Each radio installation must meet the requirements of the Federal Communication Commission and the International Convention for Safety of Life at Sea. Vessel did not have a valid Cargo Ship Safety Radio Certificate on board.

The main steering gear and rudder stock shall be capable of moving the rudder over from 35 degrees on one side to 35 degrees on the other side. Steering pump #4 limited to 30 degrees to 30 degrees

A certificate called a Cargo Ship Safety Radio certificate shall be issued after an initial or renewal survey to a cargo ship which complies with the relevant requirements of Chapter IV and any other relevant requirements of the present regulations. The Cargo Ship Radio Certificate does not match the EPIRB registration number.

A discharge of oil or oily mixture during the operation of the ship in excess of the quantities or instantaneous rate permitted in 151.10 or this subpart in 157.37 of this subchapter. Containment coaming found wasted port and starboard main deck.

An alarm system shall be provided indicating any fault requiring attention and shall be capable of sounding an audible alarm in the main machinery control room or at the propulsion machinery control position, and indicate visually each separate alarm function at a suitable position. The low combustion air for the boiler was inoperative. Perform all auxiliary boiler automatic control tests, (check point 13.0-13.1) IAW approved vital system automation periodic safety test procedures in the prescence of the attending class surveyor.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handles or touched in a normal maanner. Unapproved installation for 480 volt power in non-electrical container bays powering reefer units.

Stairways and ladders shall be so arranged as to provide ready means of escape to the lifeboat and liferaft embarkation deck from passenger or crew accommodation spaces and from spaces in which the crew is normally employed other than machinery spaces. Leader leading to the Focsle was found wasted.

The ship's name, call sign, official number, IMO international number, and principal characteristics. The ship's name and information was incorrect on the SOPEP.

Every ship engaged in voyages in sea areas beyond sea areas A1 & A2, but remaining in sea area A3 shall be provided with radio installations and equiment required by Reg 10. MF/HF radio installation was found to be inop during annual maint. The vsl is authorized to sail until such time as repairs can be made by a qualified technician. SOLAS IV/10

Fire door between E/R and ECR self closure and hinge is malfunctioning.

Rescue boat search light was inoperable.

Fire detection head in purifier room was inopperable.

Rescue boat is missing prop guard required by 4.4.6.7 of the LSA Code

One of the vessel's radar telescoping poles has 4 stays securing to the deck above the bridge by means of a welded pad eye connection which is wasted completely away and not connected allowing for this radar to move excessively, Second radar pole has three connections and one is wasted, must repair to new condition as designed.

One VHF antenna on the port aft bridge wing area is mounted by duct tape, must repair this to manufacture instructions and/or like new condition.

STBD lifeboat drum for both the fwd and aft falls is showing signs of being wasted and the steel is d-laminating, this could eventually cause the falls line to jump off the drum during operation; must prepare a permanent solution for repair to the satisfaction of the attending class society.

Vessel's cover to the port P/A connection on the exterior bridge wing is being held in place with electrical tape, this must be fixed to correct new condition.

The electrical storeroom on the navigation bridge has a wire penetration through a “A” Class boundary on the forward bulkhead in way of the door. Properly run wires through penetrations. Structural fire protection including fire-resisting divisions and protection of openings and penetrations in these divisions shall be kept in good order to ensure their required performance if a fire occurs. To be cleared by the satisfaction of the attending Authorized Classification Surveyor no later than 05 March 2018. SOLAS 74/78 Chapter II-2 Reg 14 2.1.

Fuel shut off for the emergency generator was not easily assessable, as door had to be pried open, in addition there is nothing identifying where the pull cable is located. Properly label the emergency fuel shut off on exterior aft bulkhead of emergency generator, so it is easily identified in case of an emergency. Oil fuel pipes….or daily service tank….shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. To be cleared by the satisfaction of the attending Authorized Classification Surveyor no later than 05 March 2018. SOLAS 74/78 Chapter II-2 Reg 4.2.

Air supply dampener, starboard side for the emergency generator not able to open/closed manual in case the air supply is loss of control air, all sheaves and fittings are painted over. Free up all fittings and demonstrate proper operation of dampener without control air. The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. To be cleared by the satisfaction of the attending Authorized Classification Surveyor no later than 05 March 2018. SOLAS 74/78 Chapter I Part B Reg 11

(Corrected while onboard) The electrical storeroom on the navigation bridge was being used as a storeroom (buckets, mobs, toilet paper, rock and salt coffee). By using it as a storeroom this changes the space category and does not meet the Fire Safety Objectives of SOLAS 74/78 II-2 Reg 2.1. All items had been removed while onboard.

The elevator inspection report had expired on 8/1/2017. Vessel had tried several times to get the elevator inspected serviced however the company keeps delaying the inspection. The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. To be cleared by the satisfaction of the attending Authorized Classification Surveyor no later than 05 March 2018. SOLAS 74/78 Chapter I Part B Reg 11

Vent/fire damper cut off handle safety pin for galley is rusted and frozen in place and is not easily removed in case of emergency. Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. Demonstrate proper operation of the pin and damper closure. To be cleared by the satisfaction of the attending Authorized Classification Surveyor no later than 05 March 2018. SOLAS 74/78 Chapter II-2 Reg 14.1

Replace the fuel line lagging which is worn in the starboard cargo tunnel. The condition of the ship and its equipment shall be maintained. To be cleared by the satisfaction of the attending Authorized Classification Surveyor no later than 05 March 2018. SOLAS 74/78 Chapter I Reg 11

Mechanical fitting (plug) for the containment under the fuel oil vent starboard side IWO hatch 37 is missing. Provide a mechanical fitting. Ensure the discharge containment required by §§ 154.530, 155.310, and 155.320 of this chapter, as applicable are in place. . To be cleared by the satisfaction of the attending Authorized Classification Surveyor or USCG no later than 05 March 2018. 33CFR155.320 33CFR156.120 (n) / MARPOL Annex I

The emergency light on the port side bow is hanging by lanyards and zip ties. Due to a 60% crew change out USCG was not able to witness a proper frill drill and confine space extraction drill. Prior to depart the next US port the vessel must contact OCMI to attended vessel to wittiness a satisfactory fire and confine space extraction drill. Sector Charleston [phone withheld] or [phone withheld]. In addition, if possible launch operate and recover the port lifeboat and rescue boat in water. Starboard boat were already done.

WTD gaskets in way of portside upper deck to house and CO2 room WTD entrance are damaged; replaced

10 CM Radar was reported to be not working; issued inbound LOD to make Permanent repairs prior to departing Sector Houston.
