A report of sailing short must be filed with the OCMI having cognizance for inspection in the area in which the vessel is operating,or the OCMI within whose jurisdiction the voyage is completed . MI's observed cracks in the bulkheads between the engine room and #1 generator room, #2 and #3 generator room, workshop, fuel oil purifier room, and escape trunk.

Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self-closing. Fire doors in "A" class fire boundaries in the engine room did not close properly. Additionally, multiple screws/bolts were missing in multiple "A" class boundary doors, specifically those between the engine room and generator rooms and escape trunk.

Measures shall be taken to the satisfaction of the administration to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Roughly 50% of PSTP testing has not been completed for the vessel within the last 12 months. Additionally, when conducting PSTPs for SSDG's, MI's observed that the lube oil low pressure switch activated at 2.2 bar rather than 2.5 bar and that #3 SSDG due to processor reading 4 bar instead of 5 bar.

Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship or the efficiency or completeness of its life saving appliances or other equipment, the master or owner of the ship shall report that the earliest opportunity to the Administration, the nominated surveyor or recognized organization responsibility for issuing the relevant certificate, who shall cause investigations to be initiated to determine wither a survey, as required by regulation 7,8,9 or 10 is necessary. The vessel experienced a loss of power and subsequent loss of propulsion and made proper notification to both USCG ACTEUR and FEACT however after more than 24 hours since the incident there is not evidence that the recognized origination in this case [name withheld] has been involved. As this vessel is enrolled in the MSP Select program this notification is compulsory. Make the required notifications and copy both ACTEUR and FEACT; also provide a copy of the vessels SMS that highlights reporting requirements and timelines.

Resolution: CHENG replaced the OMD and had it serviced three times. A rubber crankcase compensator vent was also found collapsed and blocked proper ventilation of the No. 1 Generator crank case. The compensator was replaced.

When in use, the lower end of the accommodation ladder shall rest firmly against the ship's side within the parallel body length of the ship and, as far as practicable, within the mid-ship half length and clear of all discharges. Vessel used a combination of pilot ladder and accommodation ladder, but did not ensure that the pilot ladder and lower end of the accommodation ladder rested firmly against the ship's side prior to taking pilots for its inbound transit to Charleston.
