Whenever a vessel is deprived of the service of a member of its complement , and the Master or Person in Charge is unable to find appropriate credentialed personnel to man the vessel , the Master or Person in Charge may proceed on the voyage , having determined the vessel is sufficiently manned for the voyage . A report of sailing short must be filed with the OCMI having cognizance for inspection in the area in which the vessel is operating,or the OCMI within whose jurisdiction the voyage is completed . In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which recognized by the administration in accordance with the provisions of regulation XI-1/1, or with applicable national standards of the administration which provide an equivalent level of safety. Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Roughly 50% of PSTP testing has not been completed for the vessel within the last 12 months. Additionally, when conducting PSTPs for SSDG's, MI's observed that the lube oil low pressure switch activated at 2.2 bar rather than 2.5 bar and that #3 SSDG due to processor reading 4 bar instead of 5 bar.

Fixed fire detection and fire alarm system installations shall be suitable for the nature of the space, fire growth potential and potential generation of smoke and gases. Multiple alarm installations, including fire alarm station in lower machinery space, were found with inoperable visual indicator.

Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship or the efficiency or completeness of its life saving appliances or other equipment, the master or owner of the ship shall report that the earliest opportunity to the Administration, the nominated surveyor or recognized organization responsibility for issuing the relevant certificate, who shall cause investigations to be initiated to determine wither a survey, as required by regulation 7,8,9 or 10 is necessary. The vessel experienced a loss of power and subsequent loss of propulsion and made proper notification to both USCG ACTEUR and FEACT however after more than 24 hours since the incident there is not evidence that the recognized origination in this case [name withheld] has been involved. As this vessel is enrolled in the MSP Select program this notification is compulsory. Make the required notifications and copy both ACTEUR and FEACT; also provide a copy of the vessels SMS that highlights reporting requirements and timelines.

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. Starboard side weathertight door near the provisions compartment would not fully seal when closed. ICLL 66/88 Annex I/12.1

Marine inspector observed port life boat lose propulsion while conducting required maneuvering evolution along side ship. Marine inspector discovered the stake pin holding the shafts together sheared in half. A temporary repair was installed. Referring to Class for final repair approval. 74 SOLAS (20) III/20.2 17C 10C

The ventilation systems for machinery spaces of category A, vehicle spaces, ro-ro spaces, galleys,special category spaces and cargo spaces shall, in general, be separated from each other and from the ventilation systems serving other spaces; the ducts are insulated to "A-60" class standard from the machinery spaces, galleys, vehicle spaces, ro-ro spaces or special category spaces to a point at least 5 m beyond each fire damper. Galley ventilation ducting was missing sealing gasket; junction box was wasted and holed; missing grounding wire. 74 SOLAS (20) II-2/7.2.1.1.4 50A

Internal combustion engines of 2,250 kW and above or having cylinders of more than 300 mm bore shall be provided with crankcase oil mist detectors or engine bearing temperature monitors or equivalent devices. Oil mist detector system control console shows reoccurring fault conditions. 74 SOLAS (20) II-1/47.2 50A

In addition to complying with the specific provisions for fire integrity of bulkheads and decks of cargo ships, the minimum fire integrity of bulkheads and decks shall be as prescribed in tables 9.5 and 9.6. Stbd side lifeboat seatbelts found stowed in a manner that does not render them able to be immediately used. 01 seatbelt was found worn and disconnected from it's mounting point. 74 SOLAS(04) III/20.2

Safe and convenient access to, and egress from, the ship shall be provided by either: a pilot ladder requiring a climb of not less than 1.5m and not more than 9m above the surface of the water, so positioned and secured that each step rests firmly against the ship's side; an accommodation ladder in conjunction with the pilot ladder in conjunction with the pilot ladder, or other equally safe and convenient means, whenever the distance from the surface of the water to the point of access to the ship is more than 9m. The accommodation ladder shall be sited leading aft. When in use, the lower end of the accommodation ladder shall rest firmly against the ship's side within the parallel body length of the ship and, as far as practicable, within the mid-ship half length and clear of all discharges. Vessel used a combination of pilot ladder and accommodation ladder, but did not ensure that the pilot ladder and lower end of the accommodation ladder rested firmly against the ship's side prior to taking pilots for its inbound transit to Charleston. Vessel's starboard lifeboat is inoperable. Cargo ships are required to carry one or more enclosed lifeboats complying with the LSA Code of such aggregate capacity on each side of the ship as will accommodate the total number of persons on board. Vessel is permitted to transit to next U.S. port of call, New York. Next port's OCMI shall determine suitability of proposed short term safety cert.

Every totally enclosed lifeboat shall be arranged that access to the lifeboat is provided by hatches which can be closed to make the lifeboat wwatertight. The starboard lifeboat's aft hatch is warped and cannot be made watertight.SOLAS 2014 cons./CH. III/31.1.1.1/LSA CODE 4.6.2.2
