Oil residue (sludge) may be disposed of directly from the oil residue tanks to reception facilities or by any other approved means of disposal of oil residue, such as an incenerator, or acceptable means which shall be annotated in item 3.2 of the Supplement to the IOPP Certificate Form A. The incenerator, annotated in the IOPP Form A, is not operational due to a burned blower motor. MARPOL (17) ANNEX I/3 Reg 12.2

Every ship shall be provided with a satellite emergency position-indicating radio beacon (EPIRB); the vessel's EPIRB battery is expired. 74 SOLAS(14) IV/ Reg 7.1.6

Where penetrations of watertight bulkheads and internal decks are necessary for access, piping, ventilation, electrical cables, etc., arrangements are to be made to maintain the watertight integrity. There is a 2" penetration in the watertight bulkheads extending from the bow thruster compartment through a fuel tank and into the forward pump room that is not properly sealed. 74 SOLAS(04 )II-1/ Reg 25-9.1

For the purpose of preventing the ignition of combustible materials or flammable liquids the ignitability of combustible materials shall be restricted; there is an excessive amount of oil in the engine room bilge due to burping the dirty oil tank into the space. 74 SOLAS(14) II-2/ Reg 4.1.3

For the purpose of preventing the ignition of combustible materials or flammable liquids, ignition sources shall be separated from combustible materials and flammable liquids; several flame screens located in the starboard vent box are of improper type (not 30X30 mesh) and or loose fitting. 74 SOLAS(14) II-2/ Reg 4.1.5

Compressed gas cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position with the valve protection cap in place. The vessel had oxygen and acetylene bottles improperly stowed on deck with rope. 46 CFR 147.60 (b)

Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. The electrical connections for the bilge alarm sensor located in the MSD space are exposed and must be enclosed in a junction box. 74 SOLAS(14) II-1/ Reg 40-1.3

THE MACHINERY SHALL BE OF A DESIGN ADEQUATE FOR WHICH INTENDED. DESIGN SHALL HAVE REGARD TO THE PURPOSE FOR WHICH EQUIPMENT IS INTENDED; VESSEL'S PORT ENGINE HAS SHUTDOWN UNEXPECTADLY AND RELIABILITY IS IN QUESTION. VESSEL WILL NOT BE AUTHORIZED TO LOAD EXPLOSIVES UNTIL DEF IS CLEARED.

The Company should ensure that each ship is manned with qualified, certificated and medically fit seafarers in accordance with national and international requirements and appropriately manned in order to encompass all aspects of maintaining safe operations on board. • The master identified one crew member with an expired TWIC (expired 29Nov2022). • One oiler’s STCW RFPEW endorsement is for vessels without a distilling plant and no waste heat or auxiliary boilers, which the ship has both. Conduct an internal safety audit and corrective action of the hiring/manning process within the SMS. Cite: SOLAS (2014) IX/Reg 3.1 ISM Code 6.2

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The bridge checklists for the fire and abandonship drills differed from the numbers in the SMS. Except as provided by § 15.725 of this part, no vessel may be navigated unless it has in its service and onboard the crew complement required by the COI. Onboard a seagoing vessel driven by main propulsion machinery of 1,000 HP/750 kW propulsion power or more, no person may employ or engage any person to serve, and no person may serve, as an RFPEW, nor may any person be designated to perform duties in a periodically unmanned engine-room, except for training or for the performance of duties of an unskilled nature, unless the person serving holds an appropriate, valid STCW endorsement issued in accordance with part 12 of this subchapter. The COI requires 3 QMEDs. One QMED’s STCW III/4, RFPEW endorsement, is for vessels without a distilling plant and no waste heat or auxiliary boilers, which the ship has both. Clear operating instructions shall be provided with a suitably worded warning notice using color coding, pictograms and/or symbols as necessary for clarity. The release mechanism operating instructions on both lifeboats are faded. Cite: SOLAS (2009) III/Reg 13.1.4, LSA 4.4.7.6.11

Waste receptacles shall be constructed of non-combustible materials with no opening in the sides or bottom. Waste receptacles found in the tunnels were plastic. Cite: SOLAS II-2/Reg 4.4.2

Logs must be established and properly maintained. GMDSS/radio station log is not completed in accordance with GMDSS logbook procedure or FCC requirements. Cite: 47 CFR 80.409

Maintain a ballast water management (BWM) plan that has been developed specifically for the vessel and that will allow those responsible for the plan’s implementation to understand and follow the vessel’s BWM strategy and comply with the requirements of this subpart. The plan must include— (1) Detailed safety procedures; (2) Actions for implementing the mandatory BWM requirements and practices; (3) Detailed fouling maintenance and sediment removal procedures. Conduct audit of the vessel's Biofouling Management Plan and make updates to logbook in accordance with plan. Cite: 33 CFR 151.2050(g)

Electrical installations shall be such that all electrical auxiliary services necessary for maintaining the ship in normal operational and habitable conditions will be ensured without recourse to the emergency source of electrical power. Faults were identified on the “Power Failure Indication Panel”. Cite: SOLAS (2009) II-1/Reg 40.1.1

A main electric lighting system which shall provide illuminations throughout those parts of the ship normally accessible to and used by crew shall be supplied from the main source of electrical power. • Lights in Reefer were filled with ice. • Lights throughout deck labeled “Watertight” lights were found with salt and/or water. All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. •Identified dead-end wires inside crane/boom rest •Dead-end wires on both port and stbd side fly-bridge •Cables, dead-end wires, junction boxes, electrical panels, and controls associated with the decommissioned Dynamic Positioning System (DPS). All electrical cables and wiring external to equipment shall be at least of a flame-retardant type and shall be so installed as not to impair their original flame-retarding properties. • Electrical connections for aft lights on fantail/stern • Identified spliced wires inside crane/boom rest (may be part of DPS) • Improper splice and insulation for cable on port bridge wing (inboard of life ring) • Port bridge wing search light cable packing gland is improper (filled with silicone). The angular position of the rudder shall be recognizable in the steering gear compartment. Local control of steering set forward of the actual rudder stocks therefore requiring the use of rudder angle repeaters in the steer space. Starboard rudder angle indicator in the steering gear compartment was off by more than 10 degrees when shifting the rudder to both side. Cite: SOLAS (2004) II-1/Reg 29.11.2

The conditions of the ship and its equipment shall be maintained to conform with the provision of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The stbd EDG exhaust drain, valve, and storage tank appear wasted and insulation missing. Cite: SOLAS (2009) I/Reg 11(a)

Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of an electrical fault in such areas, special precautions against such risks shall be taken to the satisfaction of the Administration. Unapproved open battery charging found in emergency diesel generator space. Cite: SOLAS (2004) II-1/Reg 45.5.4

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Bridge port side lifebouy was not serviceable due to crack on seam. Stbd lifebuoy needed new SOLAS retro-reflective tape. Cite: SOLAS (2009) III/Reg 20.2

The training manual…shall contain instructions and information, in easily understood terms illustrated wherever possible, on the life-saving appliances provided in the ship and on the best methods of survival. The SOLAS training manual illustrated the use of lifejackets, which showed the functionality of the lifejacket attachment strap. Two mariners attachment straps were broken. Also, identified a diagram of the ship “OIG GIANT” for the liferafts. Cite: SOLAS (2009) III/Reg 35.3

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Fire Doors were held open with tiebacks at bottom of stairway (Heavy Lift Storage Door and adjacent door to access reefers). Cite: SOLAS II-2/Reg 14.2.1.2

Means shall be provided to control leaks of flammable liquids. A small leak was observed in the vicinity of the EGEN fuel tank low tank level indicator penetration.

Inflatable liferafts shall be serviced at 12 months intervals. When impracticable, this can be extended to 17 months by the Administration. Vessel maintains fleet rafts which are rotated at routine intervals. Vessel is to service or replace rafts by next U.S. port.

Ships shall be equipped w/ rudder angle indicators to determine and display the rudder angle. Overhead, center console, and both bridge wings are all displaying different angles and in some cases approx. 10 deg different from one repeater to another.

Fire systems shall be maintained and ready for immediate use. Vessels cargo hold fire detection system is showing a "fault" due to an inoperable air flow sensor. Fire detection system was tested and was confirmed to be operational. Replace sensor to clear error.

Lifeboat falls shall be renewed and falls, winch brake, and on-load release mechanisms subject to 1.1x weight test at intervals not exceeding five years. Last five year service was completed 30Aug2014. Condition: Understanding that annual servicing was last completed 30Nov2018 and associated report indicates that all components were operationally tested and functional, the overdue five year service is to be completed no later than 30Nov2019. LR shall issue a short-term safety equipment certificate to coincide w/ above outstanding statutory requirement.

Company should establish procedures to ensure that the ship is maintained in accordance w/ relevant rules and regulations. Objective evidence indicates the vessel failed to complete 5 year testing of lifesaving applicances as required. Conduct internal audit to determine root cause of failure and implement corrective actions to prevent recurrence.

Cargo ships shall carry one immersion suit for every person on board comnplying with requirements in section 2.3 of the LSA Code. Each immersion suit shall be fitted with a light and whistle. 02 immersion suits found to be missing lights.

Fire door hold back hooks not subject to central control station release are prohibited. Self closing door was proped open while space was not in use. Remove hold back and retrain crew.

The fire training manual shall contain instruction and information required in paragraph 2.3.4 in easily understandable terms. Fire training manual missing ship specific information and not fully filled out.

Ship incinerator unserviceable from fire damage pending repairs and tests to be conducted to the satisfaction of Class Society.
