The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. Vessel's crew did not calculate the air draft and compare this to overhead hazards along the intended track as required by the SMS and the ship's bridge procedures manual. - 74 SOLAS (14) IX/3.1 and ISM Code 7 - 60ac

Every company should develop, implement, and maintain a SMS which includes the following functional requirements:... instructions and procedures to ensure safe operation of ships and protection of the environment in compliance with relevant internatioanl and flag state legistlation. The vessel's SMS has the crew calculate the air draft "taking her deepest draft into account" instead of the shallowest draft. Recommend an external SMS audit. 74 SOLAS (14) IX/3.1 and ISM Code 1.4.2
