Cylinders must always be secured and when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. PSCO observed 03 oxygen cylinders, 04 acetylene cylinders stored loose inside the acetylene locker, with an additional 05 acetylene cylinders mounted properly inside. A loose heavy cylinder cart was stored between the loose bottles, also unsecured. Additionally 02 loose oxygen bottles inside the oxygen locker with the properly secured oxygen bottles. Rope was loosely aligned over these unsecured bottles. All unsecured bottles were sitting directly on the deck. 46 CFR 147.60(b)(i)

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met:... the ignitability of combustible materials shall be restricted. PSCO observed observed oily rags and wood in engine spaces and oily rags in BSN store, which were not stored properly and are combustible materials. Additionally, a wooden crate was tied to piping on the boiler and additionally wooden crates stored surrounding the boiler. 74 SOLAS (09) II-2/4.1.3

Resolution: Received, reviewed and accepted VGP Statement and Photos from the classification society NIPPON KAIJI KYOKAI and the master stating the deficiency was rectified. The vessel conducted hull treatment in drydock before floating.

Arrangements shall be provided to enable the pilot to embark and disembark safely on either side of the ship. The securing strong points shackles and securing ropes should be at least as strong as the side ropes of the ladder. The vessels securing ropes were not as strong as the dise ropes of the ladder.

Resolution: Recieved, reviewed and accepted class report from Korea Register of Shipping stating the deficiency was rectified. Crew conducted maintenance to the engine's carburetor and is functioning correctly.
