Cylinders must always be secured and when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. PSCO observed 03 oxygen cylinders, 04 acetylene cylinders stored loose inside the acetylene locker, with an additional 05 acetylene cylinders mounted properly inside. A loose heavy cylinder cart was stored between the loose bottles, also unsecured. Additionally 02 loose oxygen bottles inside the oxygen locker with the properly secured oxygen bottles. Rope was loosely aligned over these unsecured bottles. All unsecured bottles were sitting directly on the deck. 46 CFR 147.60(b)(i)

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. PSCO observed observed oily rags and wood in engine spaces and oily rags in BSN store, which were not stored properly and are combustible materials. Arrangements shall be provided to enable the pilot to embark and disembark safely on either side of the ship. The securing strong points shackles and securing ropes should be at least as strong as the side ropes of the ladder. The vessels securing ropes were not as strong as the dise ropes of the ladder.

Document of Authorization for carriage of grain could not be presented to PSCO for examination. Vessel intends to load grain in New Orleans, Louisiana, which is the port of call after next. The International Grain Code requires a vessel to hold this document if carrying grain.
