A chemical tanker shall comply with the requirements of the International Bulk Chemical Code. Electrical installations shall be such as to minimize the risk of fire and explosion from flammable products. PSCO observed six (6) sections of corroded conduits located on starboard midship and in various locations with exposed electrical installations. 74SOLAS(20) VII/10.1, IBC Code 10.1.2.1

The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations, codes, and guidelines. PSCO observed crew unable to display knowledge of maintenance procedures or operate fixed gas detection system. The vessel/company should ensure crew is knowledgeable enable on all critical and cargo related systems. This deficiency is objective evidence of a failure of the ships SMS for fixed gas. Require an internal audit of vessel SMS focused on fixed gas detection system be conducted to the satisfaction of the OCMI within 90 days. 74SOLAS(20) IX, ISM Code 6.3

07 - Fire Safety › N/A - No Subsystem › Fixed fire extinguishing installation

The foam concentrates of low-expansion foam fire extinguishing systems shall be approved by the administration based on the guidelines adopted by the organization.(MSC.1/Circ.1312) The tests required should be performed prior to delivery to the ship and annually thereafter. PSCO observed no annual test report for the protein based alcohol resistant foam. Last service was on 07/20/2018. 74 SOLAS (Cons. 09) II-2/ 10.8.1.1 FSS Code 6.4.2.1
